Detailed Overview
In accordance with the Mission and Values of Fraser Health, the Health Care Assistant, Acute/Residential works under the direction of a professional nurse (Registered Nurse (RN), Registered Psychiatric Nurse (RPN), Licensed Practical Nurse (LPN), Nurse Practitioner (NP)), in cooperation with the interprofessional care team, and following established care plans and person-centered care.
Provides personal care to patients/residents/clients on assigned hospital units and within Fraser Hope Lodge. Duties include assisting with toileting, bathing, skin and hair care, oral hygiene, grooming, dressing, changing beds, patient positioning and mobilization. Observes and reports patient progress, assists patients with meals, transports patients, and performs related general duties. Participates in the organization and implementation of established leisure activities as outlined in the care plan; encourages resident/client participation, within the resident's/client's abilities and informs residents/clients, families and volunteers of daily plans.
Responsibilities
- Provides personal care to patients/residents/clients such as assisting with toileting, bathing, skin and hair care, oral hygiene, grooming, dressing, changing beds, patient positioning and mobilization.
- Observes and reports patient/resident/client progress including anticipated/unanticipated patient/resident/client responses to care, patient/resident/client discomfort and/or pain, skin breakdowns, and unusual incidents and/or accidents to the designated RN/RPN/LPN/NP.
- As directed by the RN/RPN/LPN/NP, takes temperature, pulse, respiration, and weight and obtains non-sterile specimens such as urine and stool.
- Utilizes mechanical aids such as lifts, wheelchairs and stretchers in order to transfer and position patients/residents/clients in comfortable and therapeutic body positions.
- Porters patients/residents/clients both within units and between units, including moving any furniture, clothing etc. associated with the move.
- Assists in the admission, discharge and transfer of patients/residents/clients including the transfer/care of clothing and personal belongings.
- Participates in the organization and implementation of established leisure activities as outlined in the care plan; encourages resident/client participation, within the resident''s/client's abilities and informs residents/clients, families and volunteers of daily plans.
- Demonstrates actions and movements to residents/clients and encourages the involvement of family members and volunteers in specific, appropriate 1:1 activities with individual residents/clients as per the care plan.
- Welcomes new residents/clients and their families by orientating them to the physical environment, routines, staff, volunteers and other residents/clients.
- Assists patients/residents/clients with meals by completing menu slips, checking meals to ensure the maintenance of dietary restrictions, serving and collecting meal trays and assisting patients/residents/clients who require help with their dining experience.
- Performs limited food preparation such as heating prepared food according to Hazard Analysis Critical Control Points (HACCP); including such activities as making tea, coffee and toast.
- Responds to call bells, determines nature of the call, assists the patient/resident/client or refers the problem to the appropriate health care team member as appropriate.
- Assists with monitoring of laundry/linen supplies and with personal laundry management as appropriate to the unit.
- Assists with body care after death, such as application of shroud and transport of body to morgue.
- Performs a variety of related general duties such as answering the phone, maintaining storage areas, ordering supplies through stores, transporting specimens, maintaining and locating inventory of equipment, cleaning and caring for equipment such as commodes/bed pans, and maintaining tidy patient/resident rooms and patient care areas.
- Attends internal/external committees, patient/resident/client care meetings and training sessions as assigned.
- Performs other related duties as assigned.
Qualifications
Education and Experience
Grade 10 plus graduation from a recognized Health Care Assistant Program or an equivalent combination of education, training and experience. Food Safety Certificate Level 1.
