Detailed Overview

Reporting to the Manager, Health Services, the Social Worker participates in the development, implementation, and evaluation of social work programs and services designed to deliver quality, client-focused care consistent with the mission, vision and values of Fraser Health.

The Social Worker acts on referrals from staff, community based agencies, self and/or family referrals, conducts comprehensive social and psycho-social assessments and liaises with community agencies to ensure optimal continuity of care for patients and residents.


Responsibilities

  1. Collaborates with the Manager, Health Services to develop, maintain and evaluate social work clinical practice standards, policies, and procedures.
  2. Assumes the clinical caseload for Fraser Hope Lodge and Fraser Canyon Hospital and acts on referrals from staff, community based agencies, self and/or family referrals. Plans, implements and evaluates treatment of clients in accordance with Fraser Health standards and the Social Work Code of Ethics. Models and facilitates a client-focused, goal-directed, interdisciplinary professional practice model. Responds to referrals from emergency and reviews admissions to acute, sub-acute, and hospice for high-risk indicators.
  3. Conducts comprehensive social work and psycho-social assessments by methods such as interviewing the patient and/or family, obtaining relevant information, gathering social data regarding the patient and/or family and interpreting the data using a variety of modalities, in accordance with professional standards and clinical policies and practices.
  4. Follows up and provides ongoing monitoring of complex psychosocial situations, evaluates patient's response to clinical interventions, follows the patient through the continuum of care.
  5. Mantains patient records by methods such as documenting patient assessments, charting patient information, preparing progress notes and reports in accordance with established standards, policies sand procedures.
  6. Acts a as a resource for patients and their families by providing information regarding community resources and access to these resources, facilitating referrals where appropriate and facilitating the development of community resources by identifyng services needed to support the health of families.
  7. Liases with hospital and community staff to clarify patient/resident and family needs. Collaborates with the interdisciplinary team to identify service gaps in the community and brings these issues to the attention of the Manager, Health Services.
  8. Maintains a variety of records such as workload statistics by methods such as recording information in approved formats, collating data, analyzing trends, accounting for variances and preparing reports in accordance with program standards and procedures. Assists with the developing and evaluating tools for ongoing outcome and performance measures.
  9. In collaboration with the Manager, Health Services, identifies, implements, and evaluates quality improvement initiatives, work methods, procedures and systems, and initiates changes as appropriate.
  10. Participates in the development and evaluation of interdisciplinary policies and procedures. Participates in the development and utilization of clinical information systems to support clinical practice and promote continual improvements in client care.
  11. Acts as a clinical consultant and mentor to members of the profession, as well as students on clinical placement from affiliated colleges or universities.
  12. Provides Critical Incident Stress Management interventions as requested.
  13. Participates in interdisciplinary and multiagency meetings and discharge planning for persons requiring complex plans.
  14. Liaises with Home Health regarding discharge planning. Provides HHC assessment and arranges provision of Home Supports in cases that require a "Quick Response".
  15. Performs other related duties as assigned.

Qualifications

Education and Experience

Master's Degree in Social Work from an accredited school of Social Work. Three (3) years' recent, clinical experience or an equivalent combination of social work education, training and experience.

Current full registration with the BC College of Social Workers. Valid Class 5 driver's license and access to a personal vehicle for business related purposes.

  • Comprehensive knowledge and understanding of counseling skills and clinical practice models as it relates to individuals, families and groups.
  • Demonstrated understanding of the scope of practice of other health care disciplines.
  • Demonstrated ability to conduct social and psycho-social assessments.
  • Demonstrated crisis intervention skills.
  • Ability to make effective referrals, reflecting in depth knowledge of community resources and strong collaborative skills in working with community health and social agency personnel.
  • Demonstrated ability to educate and provide consultation to patients, families and health care professionals.
  • Demonstrated ability to communicate effectively both verbally and in writing with patients/clients, families, staff and community agency personnel.
  • Demonstrated ability to organize and prioritize workload in a complex environment.
  • Ability to work effectively with others, including working effectively in a multicultural environment, as part of a interdisciplinary team .
  • Demonstrated ability to effectively deal with conflict.
  • Physical ability to carry out the duties of the position.
