Detailed Overview

Provides nursing care to patients according to established standards of practice and administrative policies; assesses patient's physical and psychological needs to identify problems; plans, implements and documents the care plan for an individual and/or group of patients.
Responsibilities


  1. Assesses, plans, implements and evaluates nursing care for patients and/or a group of patients based on nursing knowledge consistent with established standards of practice and hospital policies; identifies actual or potential problems and strengths; plans and performs interventions and evaluates patient outcomes; makes adjustments to care plan as required, in collaboration with members of the interprofessional team.
  2. Collaborates and consults with patients, their families and the interprofessional team regarding the patient care plan by encouraging patient and family participation, identifying issues and care solutions, explaining the care plan and resolving issues.
  3. Plans and provides teaching to patients and their families by identifying health-related learning needs to promote, enable and encourage individuals to actively participate in their care plan and accomplish their care plan goals; provides related teaching and/or refers patient to appropriate resources such as respiratory therapy, home health, palliative care and/or cardiac rehabilitation, as needed.
  4. Maintains patient records and documents care provided to the patient by collecting information such as nursing observations, interventions, teaching, care evaluations and patient responses as per established policy and professional standards of practice; reports significant change or findings to the physician, Manager or other health care provider as required.
  5. Initiates discharge planning for patients on admission; intervenes as appropriate and refers patient to other disciplines as needed; acts as an advocate to protect and promote the patient's right to autonomy, respect, dignity and access to information.
  6. Maintains a safe and therapeutic environment for patients, visitors and staff adhering to hospital standards and other policies such as infection control, fire and other emergency procedures.
  7. Participates in the planning, implementation and/or evaluation of quality improvement activities and research by identifying patient care issues and collecting relevant data; generates recommendations for alternative approaches to nursing care and identifies appropriate corrective action.
  8. Participates in the orientation and ongoing education of nursing staff and students by providing information and acting as a preceptor, as appropriate.
  9. Utilizes appropriate human and material resources in a cost effective manner in the delivery of patient care.
  10. Participates as an active member on nursing and/or hospital committees, interprofessional conferences and councils for improvement of patient care.
  11. Performs other related duties as assigned.

Qualifications

Education and Experience

Graduation from an approved school of Nursing and current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
