Detailed Overview

Reporting to the Manager, Clinical Operations, the Social Worker assists residents and their families to adjust to the stress associated with chronic illness/disability, relocation, and institutional life.


Responsibilities


  1. Completes and updates psycho-social assessments of residents and clients; participates in pre-admission assessments, as required.
  2. Provides counseling services to residents/clients, families, staff and groups; make referrals to appropriate community agencies, as required.
  3. Attends interdisciplinary care conferences and consults with others, both internally and externally in relation to resident and client services.
  4. Assists with the development, implementation and review of resident and client care plans. Records actions taken on appropriate resident and client health records.
  5. Assists in the needs assessment, development, provision and evaluation of group work for residents/clients and families.
  6. Determines residents'/clients' eligibility for assistance of a financial nature. Advises residents/clients and families, and as appropriate, makes referrals.
  7. Participates in resident and client discharge planning.
  8. Attend meetings and participate in hospital and community planning, as assigned. Assists in research projects and education programs, as assigned.
  9. Assists in the development and provision of relevant education programs for residents/clients, families and staff Social Workers.
  10. Performs other related duties as required.

Qualifications

Education, Training and Experience


Bachelor's Degree in Social Work from an accredited school of Social Work. Two year's recent, related experience working as a Social Worker in a long term care setting inclusive of clinical training and experience in individual, marital/family and group modalities of social work practices.

Current full registration with the BC College of Social Workers.

Skills and Abilities

  • Ability to deal with residents, staff and the public in a tactful and courteous manner.
  • Ability to work in a multi-disciplinary team setting.
  • Ability to work with frequent interruptions and to organize and prioritize workload.
  • Ability to communicate effectively both verbally and in writing.
  • Must possess physical ability necessary to perform the duties of the position.
