We are looking for a Family physician with advanced training in Care of the Elderly to join our team in Fraser East!
The Fraser Health Older Adult Service provides a network of clinical services including thirteen ambulatory clinic or outreach sites, four Acute Care of Elderly Units, and inpatient consulting currently at nine acute sites. Our hospitals are affiliated with the University of British Columbia and thus offer opportunities for clinical practice, as well as teaching and academic research.
The successful candidate will work in an interdisciplinary setting, working primarily with home health and homebound seniors in the Chilliwack community. Frail seniors who could benefit from further geriatric consultation are identified through the Chilliwack Home Health complex care rounds or community family practitioners. This will include home visits and liaising with the home and community care team as well as the client's family physician. Potential expansion to unattached patient consultation work is for future consideration and feasibility is to be determined. There is a potential role in transitional care (hospital to home) to improve success and decrease readmission.
This is a part-time (0.2 - 0.3 FTE) position. The successful candidate will contribute to academic and teaching components of the Service and be expected to apply for a clinical faculty appointment with the University of British Columbia.
Qualifications:
- Advanced training in Care of the Elderly (certification preferred)
- Strong collaborative interpersonal and inter-disciplinary skills
- Experience in Community Geriatric consultations
- Eligibility to apply for a UBC Clinical Faculty Appointment
