The salary range for this position is CAD $32.84 - $44.96 / hour

Detailed Overview

Reporting to the Manager, Clinical Operations or designate, performs, implements, plans and evaluates nursing care for their own work assignments within the competency and practice guidelines established by the British Columbia College of Nurses and Midwives (BCCNM), current legislative scope of practice and Fraser Health (FH) policies and procedures, within a community health setting.


Responsibilities


  1. Assesses and monitors client's actual and potential strengths and limitations including physiological, psychological, socio-cultural and spiritual needs on an ongoing basis.
  2. Provides a supportive environment for the client; contributes to the development and modification of client's plan of care; attends and provides input into interdisciplinary care planning meetings.
  3. Plans, organizes and establishes client care plan priorities when providing care; responds to unexpected events and changing client needs within scope of practice and competency guidelines.
  4. Promotes client involvement in health care decisions by facilitating teaching strategies to increase the client's understanding of care plan, chronic disease management, nursing procedures and/or interventions; reviews and evaluates teaching strategies to optimize client understanding; revises teaching strategies in collaboration with the interdisciplinary team.
  5. Performs nursing care according to identified policies and procedures, technology and Fraser Health standards; evaluates outcomes, records observations and reports problems and/or changes to designated staff.
  6. Documents and updates relevant care and procedures information to ensure client safety.
  7. Administers medications and/or provides medication reminders to clients consistent with the established policies and procedures.
  8. Assists clients with activities of daily living such as feeding, patient transfers, bathing, dressing, skin care, toileting needs; oversees client exercise routines, as applicable.
  9. Participates in quality improvement and program activities to maintain consistent level of care and to ensure that interdisciplinary team members work collaboratively.
  10. Maintains a safe working environment for the client and others by adhering to established safety policies; reports unsafe conditions and/or malfunctioning equipment.
  11. Assists in the orientation and education of new staff, students and other members of the interdisciplinary team, in collaboration with the Manager or designate.
  12. Performs other related duties as assigned.

Qualifications

Education, Training and Experience

Grade 12, plus graduation from an approved program of Practical Nursing, or an equivalent combination of education, training and experience.

Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM) and current certificates in CPR Level C or equivalent and First Aid.

May Require: Valid BC Driver's License and access to personal vehicle for business related purposes.
