Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Coordinator supervises the daily activities of the assigned Home Health Service Line and designated staff. Provides work direction to staff, coordinates staffing coverage, provides clinical leadership, guidance, and support to team members. Acts as a clinical resource on complex cases, facilitates and coordinates discharge planning, and assists staff to provide quality care by providing information based on specialized theory/practice, assessing clinical skills and/or assisting staff with activities as required.


Responsibilities

  1. Supervises nursing staff and coordinates the work of the clinical team on a day-to-day basis; observes work and corrects where necessary, coordinates and monitors the work flow of assigned staff, including prioritizing requests, assigns and adjusts work assignments and schedules, monitors and may authorize staff hours of work, including overtime, vacations, leave requests, ensuring service levels and staffing requirements are met.
  2. Applies established criteria and prioritizes referrals and assessments; leads and facilitates actions related to discharge planning and appropriate patient placement/referral as required (e.g. after hours). Collaborates with care teams by reviewing discharge planning of patients and individual care plans to ensure optimal progression to discharge.
  3. Acts as a clinical resource by consulting with staff on complex cases and demonstrating clinical expertise; provides relevant clinical consultation and direct Home Health Service Line services as necessary.
  4. Liaises with referral sources for Home Health or community services to ensure a smooth transition to Home Health from the hospital or community.
  5. Supports quality patient care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff in the use of standardized assessment tools and participation in discharge planning.
  6. Acts as a mentor and resource to staff, role modeling best clinical practices, assessing clinical skills and by coaching staff and/or liaising with other health professional leaders to help them achieve established clinical performance goals.
  7. Initiates/leads/supports plan of care conferences and rounds (e.g. iCare) for the purpose of optimal patient care management according to best practice; disseminates clinical information to nursing/nursing support staff and other members of the clinical team.
  8. Identifies learning requirements of staff and, in conjunction with the Team Leader, Manager, or Educator, provides input into the development of learning objectives; provides clinical support and guidance and evaluates the effectiveness of learning opportunities.
  9. Provides input into the development of a comprehensive orientation program, in collaboration with the Team Leader and Educator and other clinical staff.
  10. Participates in quality improvement and risk management activities by methods such as evaluating effect of changes in practice, generating recommendations for alternative approaches to practices, conducting safety audits, and supporting implementation of teaching/learning when required.
  11. Assists the Team Leader and Manager by attending panel interviews, providing recommendations on the selection of new staff, providing feedback on staff performance, and providing feedback on optimal use of budgetary resources and regarding minor and capital equipment needs as requested.
  12. Promotes staff awareness and involvement in research/evaluation activities and special projects and participates in research and special projects as appropriate.
  13. Collaborates with internal and external partners, participates on a variety of designated professional or operational committees as assigned.
  14. Participates in the review, development, implementation and evaluation of evidence-informed clinical decision support tools/policies.
  15. Performs other related duties as assigned.

Qualifications

Education, Training & Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years of recent, related clinical experience within community access and client assessment including one (1) year of supervisory experience, or an equivalent combination of education, training and experience.

Valid BC Driver's License and access to personal vehicle for business related purposes.

Skills & Abilities

  • Clinical proficiency/expertise in gerontology, geriatrics and the needs of adults living with chronic illness.
  • Comprehensive knowledge of completing client assessments and utilizing standardized assessment tools.
  • Comprehensive knowledge of broad health care services, community access and the continuum of care.
  • Comprehensive knowledge of relevant acts, legislation, standards and policies governing access to and the operation of long term care homes, assisted living facilities and other community resources.
  • Ability to supervise and provide leadership and work direction.
  • Ability to coordinate work assignments, in collaboration with staff.
  • Ability to monitor staff workload and patient factors and adjust assignments accordingly.
  • Ability to establish priorities for services, discharge planning and overall daily operations of the service line.
  • Ability to monitor the quality of patient care and ensure maintenance of standards for nursing practice.
  • Ability to monitor, assess and address performance issues based on professional standards of practice and to provide input to the Team Leader/Manager on formal performance reviews.
  • Ability to role model, coach and mentor best practices.
  • Ability to identify individual or group learning priorities to meet identified program, organizational and learner needs, in collaboration with other clinical staff.
  • Ability to support implementation of education based on changing practices e.g. legislation and technology.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to work independently and in an organized and self directed manner.
  • Ability to problem-solve and effectively deal with conflict situations.
  • Knowledge of ability to utilize applicable clinical information systems (e.g. Gold Care, Strata Pathways, Paris, Meditech ITS).
  • Ability to use Excel, Adobe Pro, Microsoft Outlook and other relevant software applications as required.
  • Ability to type 40 wpm.
  • Physical ability to perform the duties of the position.
