Detailed Overview

Reporting to the Clinical Coordinator, the Licensed Practical Nurse practices within the Scope of Practice and Professional/Practice Standards established by the British Columbia College of Nurses and Midwives (BCCNM) and within Fraser Health (FH) policies and procedures. Within an interdisciplinary health services team and collaborative nursing care model, the LPN provides direct care to stable and predictable clients seeking urgent care for mental health and substance use issues. May provide services on an outreach basis within the community where the patient is located as required.


Responsibilities

  1. Establishes therapeutic relationships with clients and families. Assesses, identifies and monitors client's actual and potential strengths and limitations including physiological, psychological, socio-cultural and spiritual needs as needed.
  2. Provides a supportive environment for the client; contributes to the development and modification of client's plan of care by identifying client priorities/plans and suggests revisions of the nursing care plan as required for stable clients; attends and provides input into interdisciplinary care planning meetings.
  3. Plans, organizes and establishes client care plan priorities when providing care; responds to unexpected events and changing client needs within scope of practice and competency guidelines.
  4. Promotes client involvement in health care decisions by facilitating teaching strategies to increase the client's understanding of care plan, chronic disease management, nursing procedures and/or interventions; reviews and evaluates teaching strategies to optimize client understanding; revises teaching strategies in collaboration with the interdisciplinary team.
  5. Performs a variety of nursing care according to identified policies and procedures, technology and Fraser Health standards; evaluates outcomes, records observations and reports problems and/or changes to designated staff.
  6. Delivers medication consistent with the written care plan, physician orders, in accordance with standards of BCCNM and established procedures; assess their effectiveness, and documents all client medications.
  7. Assists clients with activities of daily living such as patient transfers, dressing, skin care; oversees client exercise routines, as applicable.
  8. Maintains complete and accurate computerized and paper records through methods such as tracking results/consults documentation, and inputting data and documentation in accordance with established procedures and policies appropriately on the clients’ health record chart.
  9. Maintains a safe working environment for the client and others by adhering to established safety policies; reports unsafe conditions and/or malfunctioning equipment.
  10. Assists in the orientation and education of new staff, students and other members of the interdisciplinary team, in collaboration with the Clinical Coordinator.
  11. Participates in quality improvement and program activities to maintain consistent level of care and to ensure that interdisciplinary team members work collaboratively.
  12. Performs other related duties as assigned.

Qualifications

Education, Training & Experience

Graduation from a recognized program for Practical Nurses.

Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Successful completion of additional education where Mental Health and/or Substance use are the primary diagnosis as outlined by BCCNM. One year recent, related experience in substance use and/or mental health care setting, and/or an equivalent combination of education, training and experience.

Current certificates in CPR Level C or equivalent and First Aid.

May Require: Valid BC Driver's License and access to personal vehicle for business related purposes.

  • Demonstrated knowledge of principles of mental health treatment within a client and family model of care.
  • Demonstrated knowledge of other facilities and mental health resources.
  • Comprehensive knowledge of other health care disciplines and their role in client care.
  • Demonstrated knowledge and practice within the standards dictated by the BCCNM for Licensed Practical Nurses.
  • Demonstrated knowledge of suicide and violence risk assessments.
  • Demonstrated knowledge of common symptoms of various mental health diagnosis.
  • Demonstrated ability to deal effectively with conflict situations, manage aggressive behaviour and use violence prevention and behavioural management techniques.
  • General of knowledge Trauma Informed Practice and Harm Reduction approaches and Strengths-based care.
  • Demonstrated ability to promote Indigenous Cultural Safety.
  • Demonstrated ability to provide competent and culturally safety care in a variety of settings and with diverse populations.
  • Demonstrated knowledge and ability to utilize recovery oriented models of care.
  • Demonstrated ability to establish and maintain rapport with patients.
  • Demonstrated ability to observe and recognize changes in clients.
  • Ability to organize and prioritize.
  • Ability to work under the pressure of interruptions and adapt to changes.
