This position brings life to the mission of The Salvation Army in Surrey with casework services and life skills programming through the Pathway of Hope initiative. Pathway of Hope is an integrated mission case management framework, and the work of this position supports the organizational desire to move from transactional distribution services toward relational and transformational ministry. Through a referral process, engages with participants in public settings or in homes, as appropriate. The Pathway of Hope Caseworker, along with other local team members will journey with participants to reach their goals and build hope for the future. This position supports ministry unit integration and encourages the delivery of spiritual care services and life skills programming by the ministry unit team and promotes opportunities for corps members and volunteers to get involved with supporting families in our community.

KEY RESPONSIBILITIES:

Community Networking and Partnerships:

  • Form an integral part of the local Pathway of Hope team including Officers, employees, congregation members, volunteers, and community partners, to work with participants from a holistic perspective
  • Network with other service providers and maintain relationships with community agencies and local schools to provide comprehensive services for participants in conjunction with the Integrated Missions Supervisor
  • Ensure program policies and procedures are followed to maintain integrity, accountability, and program outcome measurements, attend and participate in trainings and meetings as required by the Ministry Unit, Divisional and Territorial Headquarters
  • Connect with the local team weekly and the THQ Pathway of Hope Regional Coordinator (virtually or in-person) monthly to discuss participant progress and team functioning
  • Assist with programming as needed which may include training opportunities and engagement in various programs including Red Cap/ TASK anger management programs, CAP Money Course, as well as the set-up, facilitation or clean-up after programs
  • Support with seasonal services (e.g. intake for Christmas assistance, Christmas kettles, Santa Shuffle, and summer camp)
  • Performs other duties as assigned

Direct Case Management Services:

  • Engage and build rapport with the target population building strong relationships
  • Perform all aspects of the case management process using the Pathway of Hope framework (pre-screening, intake, assessment, goal planning, action, transition, and follow-up)
  • Develop goal plans with each participant and schedule regular meetings to review progress and engage in further planning
  • Complete initial and ongoing assessments with each participant every 3 months
  • Effectively maintain records in Link2Feed to accurately reflect services rendered using all appropriate forms, including detailed case notes for every interaction
  • Ensure confidentiality is maintained in accordance with Operating Policy and applicable legislation
  • Conduct/arrange home visits, as needed
  • Provide information and referral services, as needed, assisting participants in making linkages and accessing community resources; offer/arrange accompaniment services as appropriate

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

Education/Certifications:

  • Post-secondary degree/diploma in Social Work or another Human Services Field of at least 2 academic years is required
  • Current certification in First Aid/CPR or willingness to obtain
  • Note: Alternative combinations of education and experience may be considered.

Experience:

  • Minimum 2 years of experience working with social service agencies and clients or previous casework experience
  • Proficient in Microsoft Office and Microsoft Teams applications
  • Experience with Canva, social media and Link2Feed is an asset
  • Ability to enter data and perform statistical analysis efficiently and accurately
  • A valid Driver's License and a clear Drivers Abstract
  • Previous experience in casework, social services, or mental health setting
  • Display competence in interpersonal skills, developing and maintaining appropriate boundaries in client relationships and building interagency relations
  • Excellent organizational, administrative, and communication skills (written and oral)
  • Have experience with program facilitation and program development
  • Training in trauma-informed care, non-violent crisis intervention, mental health first aid, motivational interviewing is considered an asset
