SUMMARY OF DUTIES: Under the direction of the Nursing Coordinator, or designate, is responsible for the care of residents in keeping with the philosophy and policies of Extendicare and carrying out nursing, resident care, and administrative duties in the Nursing Department as directed. Utilizing a primary model of care the LPN will provide resident care which includes assessing developing care plans; monitoring; measuring outcomes, reporting; updating professional competence; participating in client focused teams with other healthcare providers. Advocating for clients including communicating client needs to healthcare stakeholders; reporting on abuse or unsafe conditions. Educating and training resident's and families on health care issues; teaching residents and families to use assistive devices; raising awareness of community services. Supervision and monitoring including teaching, delegated acts to health care providers (professional and support staff); Monitoring peers and preceptor ship (new LPN's to students). Information management including documentation, producing initial, interim, and, if needed, discharge reports. This also includes Resident Care Plans. QUALIFICATIONS: 1. Currently licensed with the College of Licensed Practical Nurses of Manitoba. Possesses sufficient practical experience to satisfactorily perform the responsibilities of the position. 2. Mentally and physically able to perform duties as outlined. 3. Knowledgeable in the principles and philosophy of long term care. 4. Ability to supervise and direct the work of others. 5. Demonstrates concern and interest in providing quality care to residents. 6. Ability to work independently with minimal supervision and complete assignments in a timely manner. 7. Ability to communicate with other staff, residents, families, and the public. 8. Experience 1 year in a Long Term Care setting, 1 year clinical, supervisory experience. SWAT certification obtained from WRHA/equivalent or to be obtained with employment. 9. MDS/computer skills.
TYPICAL DUTIES AND RESPONSIBILITIES: 1. In collaboration as required: • assists in the development and maintenance of care standards and objectives; • assists in the development of an ongoing review of resident care plans which include rehabilitative and restorative objectives. 2. Supervises Health Care Aide staff and performs direct resident care in accordance with policy and procedures. Makes regular rounds, performs treatment services as required. 3. Maintains resident health care records and performs charting duties ensuring that records are informative, accurate, and current. 4. Admits new residents and orientates new residents and their families to all aspects of the facility, as per the admission procedure. 5. Participates in regular reviews of resident care plans with members of the health care team. 6. Participates in the orientation of new staff and the supervision of Health Care Aide's on the unit. Promotes and participates in ongoing staff education. 7. Monitors and reports changes in the resident's physical or mental condition to the Nursing Coordinators/delegates. 8. Orders prescribed medications, transcribes physician orders, ensures physician's telephone orders cosigned, and regularly reviews treatments with attending physicians. 9. Assists in scheduling resident appointments, including visits to the Podiatrist and other visiting professionals. Arranges transportation for residents to appointments. (Prepares transfer forms as required.) 10. Monitors the use and maintenance of supplies and equipment in the Nursing Department. 11. Performs all duties in accordance with the written policies and procedures of Extendicare, the facility, and the Nursing Department
