JOB PURPOSE

The Human Service Counselor assesses, supports and assists clients / patients within the community or hospital-based setting through the implementation of therapeutic and rehabilitative programs.

QUALIFICATIONS

  • High school graduation plus an additional human services program/course of over six months and up to one or post-secondary training in a related field or equivalent.
  • For community based positions in Additions and Mental Health must have 1 year of related experience and posses a valid license.
  • Ability to attend work on a regular basis.
  • Ability to perform the duties of the position.
  • Good work record.

KEY RESPONSIBILITIES

The Human Service Counselor is responsible for:

  • Participating in the assessment, development and implementation of the client support services plan.
  • Observing, documenting and reporting, physical, behavioral and psychological changes in the client.
  • Reviewing, evaluating and modifying individual service plans as part of the multidisciplinary team.
  • Intervening in crisis situations and providing emotional support to clients.
  • Assisting clients with activities of daily living by teaching and counseling on lifestyle skills such as parenting, financial planning.
  • Transporting and accompanying clients to activities and/or appointments related to their care.
  • Organizing and implementing individual and group therapeutic programs
  • Assisting clients to access services of various government and community agencies.
  • Showing others how to perform tasks or duties as part of the departmental orientation.
