Who you are
You have Bachelor's Degree in healthcare discipline or related field (ie: Health Management) and must have a minimum of 5 years of supervisory experience in health care. You have proficiency level in Microsoft Office applications.
- Demonstrated skills in facilitating, team building, and conflict resolution
- Demonstrated ability in planning, budgeting, information analysis and reporting
- Experience negotiating services and resources between programs and partners
- Demonstrated knowledge of community partners and resources
- Demonstrates excellent communications and interpersonal skills
- Demonstrated experience in handing issues and information of confidential and sensitive nature
- Ability to attend work on a regular basis.
- Ability to perform the duties of the position.
- Good work record.
Written and spoken competence in English and French is required at an advanced level.
What you will do
- Plan and evaluate overall operations and programming for Health Centres
- Overall responsibility for budget and allocation of resources to respond to Health Centres' needs/ population served
- Work with designated lead at each Centre to ensure appropriate staffing
- Sit on the Community Advisory Committee (or as delegate)
- Is responsible for issues related to staff: e.g Attendance Management and/or progressive disciplinary processes
- Ensure clinical practice guidelines, nursing standards, and community standards are met
- Collaborates with providers and staff at community health centres to ensure access to appropriate provider is timely (based on Third Next Available reports, roster size and patient visit/provider
- Facilitates collaborative approaches within interdisciplinary team as to provide comprehensive care
- Supports mental health population health and chronic disease prevention management practices within the teams
- Identify new services requirements and deficiencies in current services and will work with Primary Health care Coordinator, Community Advisory Committees, and others to develop implementations plans
- Participates in recruitment including job postings, interviews, and orientation
- Accepts requests for education and approves based on Horizon policy
- Reports activities of all Health Centres monthly to Director in regard to current status and future plans
- Completes Performance Appraisals for staff annually
- Works with Medical Liaison(s) and Clinical Department Head of Family Medicine to ensure open communication between medical staff and Horizon
- Maintains good knowledge of community resources in order to better meet client needs
- Will work with Primary Health Care Access Coordinator and other programs such as Mental Health and Addictions to ensure integration of resources and programming to respond to identified needs of community
- Conduct team meetings for direction and support
- Serves on Community Health Centres Needs Assessment work groups, as appropriate
- Manager is responsible to support the Nurse Practitioners of this area
