JOB PURPOSE
The LPN provides Primary Health Care services, Illness/Injury Prevention, Chronic Disease Management, and Community Development services, using a population health approach in an interdisciplinary team of health providers. In keeping with the clinic plan, the provincial CHC framework, and the Community Health Nursing Standards, the service model is based on the principles of primary health care with the primary goal of supporting communities in meeting their health care needs.
As a member of the interdisciplinary team, the LPN provides safe, competent, ethical and compassionate care to patients and families. The LPN performs duties and responsibilities under the direction and in collaboration with a Registered Nurse or duly qualified medical practitioner or pharmacist.
QUALIFICATIONS
- Graduation from an approved school for Licensed Practical Nurses.
- Must be currently registered with the NB Association of Licensed Practical Nurses.
- Ability to work within a multidisciplinary team and a collaborative practice work structure.
- Ability to function within the LPN scope of practice.
- Demonstrates initiative, good judgment and the ability to set priorities.
- Consistently demonstrates effective communication skills both verbal and written.
- High regard for confidentiality.
- Relevant work experience as a Licensed Practical Nurse within the past five years.
- Willing to complete any relevant advanced competency education.
- Maintains annually, CPR and other online learning requirements
- Ability to attend work on a regular basis.
- Ability to perform the duties of the position.
- Good work record
KEY RESPONSIBILITIES
Patient Care
- As a member of the interdisciplinary team provide quality patient and family care in a primary care clinic environment within scope of LPN practice.
- Patient care responsibilities and duties include but are not limited to specimen collection and preparation for transport; in house lab testing i.e. glucometer readings; performing patient vital signs and other assessments; collecting and reporting pertinent patient information; collaboration on patient care plans and documentation as per nursing standards.
Community and Primary Health
- Participates in the planning and programming of clinic services through active involvement on committees and sharing information about perceived health needs and problems within the community
- Collaborates with the interdisciplinary team in the development and implementation of community clinics, i.e., Flu Campaigns.
- Maintains good public relations and promotes a collegial and collaborative working relationship with CHC staff, volunteers, patients and partners in the community.
- Actively participates in planned primary health care programs.
Communication and Professional Accountability
- Promotes positive relationships with patients, families and community partners.
- Works collaboratively with members of the interdisciplinary team.
- Promotes effective inter-departmental and facility communication.
- Communicates effectively with patients, families and members of the interdisciplinary team with a high regard for confidentiality.
- Communicates clinical/administrative concerns to Facility Administrator or designate.
- Demonstrates commitment to work in a professional manner in the workplace.
- Assumes responsibility for renewing and maintaining active licensure with their provincial regulatory authority annually
Education
- Attends in-services and continuing education programs for personal development and participates in formal educational programs.
- Willingly shares nursing-related knowledge and skills with other staff members.
- Assumes responsibility for maintaining clinical competence/certification including annual
- Participates in the orientation of new staff.
