Horizon Health Network in beautiful New Brunswick is looking for a Primary Health Care Patient Navigator to join our team!
Horizon Health Network (Horizon) is seeking a Primary Health Care Patient Navigator to work within a collaborative, team-based primary healthcare centre, partnering with interdisciplinary team members to support patients in navigating the healthcare system and accessing community resources. The Patient Navigator will serve as a critical resource and a primary point of contact for patients and their families, facilitating of patient and family centered care within a population health framework. This role is instrumental in guiding patients through the complexities of the New Brunswick healthcare system, facilitating timely access to healthcare services, and connecting them with community resources.

Who you are
- Degree in a related health or social sciences discipline preferred, or equivalent experience in community health programming or health/community services.
- 1-3 years of experience in health care and community engagement or patient navigation.
- Strong understanding of the social determinants of health and experience with community health programming.
- Excellent interprofessional collaboration and communication skills, including verbal, written, and electronic communication.
- Demonstrates knowledge and skills in assessing the patient and their family's health and social needs in a community setting.
- Experience working with different cultural/ethnic groups and backgrounds.
- Experience working with diverse populations and individuals with chronic health conditions is an asset.
- Knowledge of healthcare terminology or a willingness to learn.
- Competence in providing services using various virtual care methodologies.
- Demonstrated cultural safety, humility, and competency in healthcare settings.
- Proven ability to build trusting relationships and facilitate referrals.
- Familiarity with community resources that impact the social determinants of health.
- Work to full scope of knowledge and competencies.
- Good work and attendance record.
- Ability to travel.
Written and spoken competence in English is required at an advanced level.
What you will do
- Connecting patients with healthcare providers and community services, ensuring that patient needs and preferences are addressed in a timely manner.
- Providing ongoing support and guidance to patients and their families, helping them overcome barriers to healthcare and social systems. This includes assisting clients with accessing processes, such as navigating registration processes and forms.
- Provide navigation services in-person at health centre and virtually/ via telephone.
- Educate patients on services available within primary health care and the continuum of care and empower them to manage their health with the help of community resources.
- Fostering respectful and supportive relationships within the interdisciplinary healthcare team and with community partners
- Collaborating with healthcare professionals, community organizations and municipalities to enhance patient care experiences, and improve access to primary healthcare services.
- Engaging with the Tantramar community to strengthen networks and resources and inform the community of primary health care resources.
- Using a patient and family-centered approach in all interactions, demonstrating respect, compassion, and understanding of social determinants of health.
- Developing and maintaining educational materials that are culturally sensitive, linguistically appropriate, and tailored to the health literacy levels of the patient population.
- Staying updated on local, regional, and provincial healthcare resources and best practices.
- Keeping informed about emerging trends in community health programming.
- Identifying and addressing the gaps in health and community/social resources.
- Participating in professional development opportunities to enhance cultural competency and health literacy.
