Reporting to the Primary Health Care Lead, the incumbent will provide expert knowledge and skills on a wide range of issues from advocacy to assessment to case management. As a member of a Primary Health Care (PHC)team, the PHC Dietitian provides a range of PHC services. These services include, but may not be limited to, health promotion, illness prevention, screening, treatment and monitoring of people living with a variety of chronic illnesses. Expertise is required to support people living with chronic health problems such as diabetes, hypertension, chrohn's, colitis, arthritis, dyslipidemia, and others. The incumbent will be required to collaborate with members of the interdisciplinary health team to provide high quality, safe, and ethical patient care. These services are provided within a holistic, patient/family centred care framework.
The incumbent will provide health assessment, interventions and appropriate follow up in accordance with acts, regulations, policies, standards, guidelines, mission and objectives of the Registered Dietitians of Canada, the Nova Scotia Dietetic Association, the Department of Health and Wellness - Province of Nova Scotia and the Nova Scotia Health.

$32.95 - $43.42 Hourly
