About the Opportunity

Under the direction of the Program Manager for the Rehabilitation Ambulatory Care Clinics, the Occupational Therapist position is posted with defined responsibilities related to helping our patients with transition planning (e.g., community re-integration, return to school or work, etc.), resource navigation, resource counseling, case coordination and community liaison support. This requires excellent skills of advocacy, communication and navigation.

The Occupational Therapist is attentive to directing patients screening and prioritizing outpatient Occupational Therapy referrals, providing consultation and assessment, and follow-up of clients in the occupational performance areas of self-care, productivity and leisure. The objectives of treatment are to facilitate, restore and/or maintain clients' optimal level of function within the community. The Occupational Therapist provides intervention to address clients' occupational performance components (physical, cognitive and/or affective). The Occupational Therapist provides education and support to clients, families and caregivers. The Occupational Therapist functions as a member of a multidisciplinary hospital and/or community health care team.


The Occupational Therapist has self-initiative and tenacity to establish and maintain relationships with relevant healthcare community agencies, medical equipment suppliers, private rehabilitation professionals, employers, educational and informational technology resources, funding agencies, government agencies and relative insurance representatives.

The Occupational Therapist has leadership skills and abilities to manage a complex caseload, prioritize the delivery of care in a timely fashion to a large community population.

The Occupational Therapist may be required to provide services throughout the Rehabilitation and Supportive Care Services portfolio.

About You

We would love to hear from you if you have the following:

  • Graduate of a C.A.O.T recognized School of Occupational Therapy
  • Registered with the College of Occupational Therapists of Nova Scotia (COTNS)
  • Current BCLS certification
  • Experience collaborating with community partners and patients through liaising, navigating, follow-up and client evaluation to ensure measurable positive outcomes
  • Travel to various sites within the community will be required
  • Experience working with patient with complex chronic health conditions and long term pain preferred
  • Experience co-facilitating education sessions with other multi-disciplinary team members preferred
  • Demonstrated effective communication and interpersonal skills
  • Competencies in other languages an asset, French preferred

Hours

  • Permanent, Full-time position; 75 hours bi-weekly

Compensation and Benefits

$35.86 - $45.29 Hourly
