About the Opportunity

The Community Health Team (CHT) is a community-based health initiative that will focus on wellness programming and wellness navigation and working with community partners. The CHT works in collaboration with other groups/organization in the community to offer programming and support. CHT wellness programming will include risk factor management, healthy eating, physical activity, mental wellness and parenting

Reporting to the Health Services Manager, the Wellness Facilitator (Physiotherapist) demonstrates flexibility and innovation in supporting citizens of the communities that the CHT serves in accessing the services, programs, and/or resources that can support health enhancing behaviors. They provide guidance and support for clients, including providing information about appropriate supports and programs that will meet health and wellness needs and social supports that can assist with daily living. They lead support for a group based low intensity exercise program, as well as other physical activity programs offerings within the community. The individual is responsible for exercise program development, implementation and evaluation and must possess excellent exercise leadership skills.

The Wellness Facilitator is team focused and will collaborate with the CHT staff, primary care providers, other health care providers and community groups to maintain awareness of existing supports within the community and in the surrounding areas of the district through a community development, population health and collaborative approach. The incumbent is a member of an inter-professional team and collaborates with colleagues to determine team goals and objectives, participates in the evaluation of team effectiveness, solves problems, and shares responsibilities. Excellent interpersonal and written communication skills are required.

About You

We would love to hear from you if you have the following:

• Bachelor's degree in physiotherapy from an accredited university required
• Membership with the Nova Scotia College of Physiotherapists required
• Must be eligible for membership with the Canadian Physiotherapy Association
• Minimum one year recent experience in each of the following: working with people with chronic disease in an exercise program or setting; working with community groups/partners building trusting relationships through working together on projects/initiatives/partnerships that benefit the public (professional and/or volunteer); Individual and group education/facilitation utilizing adult education, self-management supports, and motivational enhancement principles
• Understanding of population health, health promotion strategies, and behaviour change principles
• In-depth knowledge of local health authority and community-based services.
• Demonstrates person-centered/family-focused, diversity focused approaches to care
• Well-developed interpersonal, teamwork, oral and written communication skills
• Demonstrates flexibility, adaptability and willingness to change

• Demonstrates innovation in program planning, implementation and evaluation, and quality improvement, planning skills
• Computer skills (MS Office, e-mail and internet)
• Annual BLS AED certification and annual WHMIS training and certification
• Travel to other sites is required. Valid Nova Scotia driver's license and daily access to a reliable vehicle is required
• Competencies in other languages preferred; French an asset

Compensation and Benefits

$35.89 - $42.89 Hourly
