About the Opportunity

Reporting to the Western Zone Patient Flow Manager, the Patient Flow Coordinator is responsible for identifying opportunities for improvement in advancing patient care plans which support timely discharge, patient flow and resource utilization throughout the Western Zone using the Bed Utilization Management System Software.

The Patient Flow Coordinator ensures that the principles and practices that guide Nova Scotia Health such as the mission, vision, values, expected behaviours, leadership philosophy, organizational health, population health, ethics, safety, quality, partnerships and inter professional collaboration are integrated within the services they provide and through the messages they deliver, including:

  • Promote the use of software program for bed utilization according to established guidelines and policies
  • Liaise with Western Zone Patient Flow Manager and physician leaders as required and communicate identified processes and/or issues impeding patient flow
  • Lead for training and quality of the software system across Western Zone inpatient units
  • Collaborate with the interdisciplinary team to utilize their data and advance the plan of care for each patient, including assisting the team to meet the expected LOS targets and ensuring that a discharge plan is in place
  • Assist to identify opportunities for improvement in resource utilization and patient flow
  • Work with teams to ensure that wherever possible patients are in the right bed to support their care needs and advance the plan of care

About You

We would love to hear from you if you have the following:

  • Bachelor's degree in nursing preferred
  • Current or eligible for registration with the Nova Scotia College of Nursing
  • Certification and/or experience in the use of information technology, the analysis and presentation of data and adult education
  • Minimum of five years' of recent acute care experience
  • Demonstrated minimum of one year experience in clinical utilization
  • Demonstrated leadership abilities
  • Demonstrated accountability
  • Demonstrated critical thinking, problem solving, judgement and decision-making skills
  • Strong client focus and ability to build solid client/practitioner relationships
  • Demonstrated collaboration and teamwork skills
  • Demonstrated planning, organizational, prioritizing and time management skills
  • Competencies in other languages an asset, French preferred

Hours

  • Full-time Long Assignment position; 75 hours bi-weekly
  • Approximately 12 months
  • Successful applicant may be required to work flexible hours including days, evenings and weekends

Compensation and Benefits

$41.58 - $51.11 hourly
$52.90 with 25 years' RN experience
