About the Opportunity
The Dietitian reports to the Health Services Lead of Primary Health Care and works within a collaborative multi-disciplinary team to provide advanced clinical skills and techniques to the population of patients served by the Collaborative Family Practices within Pictou County. The Dietitian acts as a resource in the area of nutritional assessment, education and counseling in both one on one and group settings. The Dietitian is responsible for accessing, consulting and mobilizing available interdisciplinary and community resources to facilitate client goal attainment.
As part of the One Door Chronic Disease Management Team, the dietician uses clinical practice guidelines to provide nutrition care and education to patients/clients with a variety of clinical conditions that require nutritional intervention. The dietician will deliver services to a variety of patient populations including high risk groups suffering from chronic diseases and/or complex multi‐system diseases. This role will also provide health education to patients with a focus on health promotion, disease prevention, and chronic disease management. The CDM Dietician is a part of the Primary Health Care Program and reports to the Primary Health Care manager.
This position provides services in primary care centers throughout the Pictou County area, travel will be required and as such access to reliable transportation is required.
About You
We would love to hear from you if you have the following:
- Bachelor of Science in Human Nutrition from a recognized University required
- Completion of Dietetic internship (a 1 year integrated or post graduate program) required
- Licensure by Nova Scotia Dietetic Association required
- Membership in Dietitians of Canada preferred
- Certified Diabetes Educator preferred
- Two (2) years' experience in an adult health care facility preferred
- Knowledge and proficiency in current, evidence-based methods and practices in the field of nutrition, with an emphasis on the prevention and management of chronic disease
- Strong understanding of health promotion, population health and the determinants of health theory and practice would be an asset
- Experience with or willingness to learn group facilitation, adult education and motivational interviewing techniques
- Understanding of or willingness to learn health coaching techniques and intervention
- Experience in using an EMR an asset
- Demonstrated competence in electronic communication, excellence in Microsoft Office products, including access and word

Permanent, Full-Time (75 hours bi-weekly)

Compensation and Benefits
$34.78 - $45.85 Hourly
