About the Opportunity

Reporting to the Manager of Clinical Pharmacy Services and the Seniors and Frailty manager, the pharmacist is responsible for ensuring safe and effective drug therapy management and optimization for geriatric patients presenting with polypharmacy and geriatric syndromes. This opportunity involves delivering pharmacy services in both the emergency department and patients' homes. Working in the Seniors and Frailty team, the pharmacist participates in all aspects of care for geriatric patients including medication optimization, immunizations, prescribing/deprescribing, pharmacokinetic monitoring, chronic disease management and is a steward for all appropriate medication use. The candidate will also engage in quality committees, quality improvement and research initiatives. Further, the candidate will provide educational sessions to other health care professionals, as well as precept pharmacy learners. Work hours may include evenings and weekends.

About You

We would love to hear from you if you have the following:

  • Bachelor of Science in Pharmacy or an entry level Pharmacy Doctor of Pharmacy from a CCAPP (or similar standard) accredited university required
  • Licensed, or eligible to be licensed, with the Nova Scotia College of Pharmacists as an Active Direct Patient Care Pharmacist.
  • Completion of a Hospital Accredited Canadian Pharmacy Residency an asset.
  • Board Certification in Geriatric Pharmacology an asset.
  • Nova Scotia College of Pharmacists drug administration by injection permit an asset.
  • Evidence of skills and experience in applying the principles of Medication Safety and Quality / Risk Management an asset.
  • Experience with relevant research studies, including research skills for the safe and ethical conduct of clinical and epidemiologic research an asset.
  • Advanced clinical skills - comprehensive understanding of polypharmacy, geriatric syndromes, pharmacokinetics and dynamics of older adults
  • Demonstrated organizational, interpersonal and communication skills, independent and collaborative decision-making skills required.
  • Mentoring undergraduate (pharmacy, medicine) students, post-graduate students (pharmacy, medicine, nurse practitioner), pharmacy staff (direct patient care pharmacists), other health care professionals
  • Competencies in other languages an asset; French preferred
  • Access to a reliable vehicle

Compensation and Benefits

$47.03 - $58.77 Hourly
