Reporting to the Senior Director, Continuing Care, the Director is accountable for the overall planning, development, implementation and evaluation of Continuing Care services and programs and stewardship of all resources allocated to continuing care at the zone level within the context of Continuing Care as a provincial program. The Director ensures continuing care expertise in home care, community based services and long term care is provided to stakeholders and clients.
The Director leads the Continuing Care leadership team at the zone level, providing strategic and operational direction to continuing care employees and working in collaboration with stakeholders across all levels of the health system. The Director for Continuing Care has oversight and responsibility for the care coordination teams in Western Zone, the provincial intake team that initially processes all Continuing Care referrals, and the Nursing Only Assessment team which provides case management services to clients requiring only in-home nursing services.
The Director ensures that services are evidence-informed and consistent with best practices while ensuring resource sustainability. The Director ensures that programs and staff comply with all professional, ethical and legislative requirements. They participate in or lead a number of committees, councils and work groups, with a range of partners internal and external to NSH, designed to promote the improvement of services and supports for individuals requiring continuing care services. The Director also collaborates with members of the Department of Seniors and Long Term Care. Specific accountabilities include: monitoring key performance indicators, leading a culture of quality improvement, contributing to continuing care policy development and implementation, sharing local issues and perspectives, and engaging in conversations about the health care system. The Director provides advice and assistance to the Senior Directors, zone Executive Directors and Vice Presidents as required.
