The Social Worker works under the supervision of the Manager of Seniors Western Zone and is a member of the interdisciplinary Seniors Care Team providing service primarily in the emergency department and community setting. The Social Worker (SW) participates in the planning, development and provision of social work services to support seniors and enhance their independence and function at home. The Social Worker works to full scope of practice as an independent practitioner within the team. This includes supporting the development and integration of seniors' health services within hospital and the community. The SW provides collaborative, client centered care across the continuum, supporting the psychosocial and emotional well-being of all individuals (includes patients, families, groups, populations and communities). This position includes considerable collaboration within the team and with community partners. Excellent interpersonal and written communication skills are required. The SW contributes to the clinical education of Social Work students and related health disciplines. The SW is responsible for adhering to Nova Scotia Health policies and procedures and to the professional standards of their licensing body, the Nova Scotia College of Social Workers.
