As a Care Connector, you will work collaboratively with complex vulnerable and priority patients and their family members/caregivers/substitute decision-makers (SDM’s) to make a successful connection with an appropriate primary care practitioner based on the needs of the patient

What will you do?

  • In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plans
  • Gathers information by working collaboratively with complex vulnerable and priority patients and their family members/caregivers/substitute decision-makers (SDM’s) to make a successful connection with an appropriate primary care practitioner based on the needs of the patient
  • Maintains contact with patients on the Ontario Health atHome priority list to keep them informed about prospects for attachment
  • Provides information about the Unattached Patients Program (UPP), patients’ rights and responsibilities and family health care services available
  • Develops knowledge of primary care services and community resources to provide useful supports to patients or other stakeholders..
  • Collaborates with patients and others in their support network to gather information to make successful connection with an appropriate family health care provider based on the patient’s care needs
  • Provides information and referral assistance to patients for assessing alternative community resources (i.e. After-hours clinics, urgent care centres, etc.)
  • Supports patients who face a longer wait time in their efforts to find an appropriate provider during the attachment process
  • Obtains consent for the fathering and sharing of patient information acquired through the attachment process

What must you have?

  • Completion of a Registered Practical Nursing program and registered with the College of Nurses of Ontario
  • Solid knowledge of patient diagnosis and matching to appropriate health care provider
  • Solid knowledge of community resources and ability to navigate within these resources
  • Knowledge of and adherence to Ontario Health atHome policies, procedures and related practices
  • Basic knowledge of and adherence to relevant legislation such as Personal Health Information Protection Act (PHIPA)
  • Basic understanding of issues and priorities within the healthcare sector
  • Strong understanding and commitment to quality service and best practice
  • Good knowledge of Ontario Health atHome patient services strategies, objective, priorities and processes
  • Good understanding of applicable databases
  • Proficient in a Windows environment
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.

What would give you the edge?

  • Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language

Temporary Full-Time Care Connector (for approximately six months with the possibility of an extension)
