As a Care Connector, you will work closely with the medical community and our Ontario Health at Home health and community partners to make appropriate connections with individuals by assisting them in searching for and finding a healthcare provider in their area.

Whether you work in our office, in the community, or in a health care facility, you will play a lead role in providing connected, accessible, patient-centred care and be supported by our collaborative team, which includes over 9,000 regulated health care and other professionals.

What will you do?

Systems Navigation/Accessing Resources and Linking

• Assesses needs and refers individuals without a regular care provider to physicians and nurse practitioners who are accepting new patients in their community
• Conducts assessments related to health care provider needs over the phone
• Prioritizes unattached patients who register for the program and enter their information in a provincial database
• Develops knowledge of primary care services and community resources to provide useful support to clients or other stakeholders
• Assures adequate documented attempts to connect clients with healthcare provider
• Collaborates with patients and others in their support network to gather information to make successful connections with an appropriate family health care provider based on the patient's care needs
• Provide statistics to the lead Care Connector on outreach activities

Community Engagement

• Builds positive, effective relationships with internal and external stakeholders, including primary care providers, Ontario Health atHome and hospitals
• Encourages health care providers to register and to participate in the program
• Responds to inquiries and requests for information and services in accordance with the individual(s) needs, identified risk factors, and urgency for services
• Provides information/education to community health providers to clients, families, community groups, hospitals and other health/social services providers through outreach activities such as presentations, attending medical conferences and panel participation

Team Building

• Develops professional working relationships with team members
• Works respectfully, positively and collaboratively within a team environment, sharing experiences and lessons learned
• Supports the team and works with team members to ensure department and/or patient/family needs are met including absence coverage

What must you have?

• Registered Nurse (RN) designation with a current registration and member in good standing with the College of Nurses of Ontario
• A University degree is preferred (or an equivalent combination of education and experience)
• Minimum two (2) years of recent experience in community health or a related field
• Working knowledge of community resources (i.e. services and programs)
• Adept in using MS Office applications (e.g., Word, Excel, Outlook, PowerPoint, etc.)
• Understanding and commitment to quality service and best practice
• Flexible, adaptable and responsive to change
• Excellent and detailed documentation skills (clear, thorough, accurate and timely)
• Valid driver's license and access to a reliable motor vehicle
• Insurance that includes driving for business purposes and minimum liability of $1,000,000.
• Ability to communicate in French or other languages an asset.

• Proficient in a Windows environment
• We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.

What would give you the edge?

• Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
• Case management experience or recent related community experience
• Ability to speak French or another second language
