This position will be in the community on the mobile H.O.M.E bus, a multi-agency team that provides health services to people where they are at: shelters, encampments, housing, and in other community settings. The Community Worker will identify and engage individuals experiencing homelessness and at-risk populations, helping them navigate and access services, referrals, and other vital resources.

Working collaboratively within a multidisciplinary team of physicians, nurse practitioners, social workers, community developers and support staff, the Community Worker will advocate for both the Health Centre and the individuals they serve. This is an impactful role that combines outreach, support, and advocacy in a community setting.

THE COMPENSATION PACKAGE

  • A starting salary range of $50,000 - $53,000
  • Monday to Friday - no weekends!
  • A Psychologically Safe and Inclusive Workplace
  • 100% employer-paid health & dental package, including up to $2,000/benefit year for mental health, $1,500 for massage therapy
  • Healthcare of Ontario Pension Plan (HOOPP)

REQUIRED QUALIFICATIONS

  • Postsecondary education in social services and/or direct experience providing education/support/counselling to communities at risk
  • Knowledge and understanding of the social determinants of health
  • Ability to work within the harm reduction model of service delivery
  • Willingness to work with people of diverse backgrounds with an inclusive and anti-oppressive lens
  • Valid G classification driver's license and driver’s abstract
  • Comfortability operating the H.O.M.E team bus

EXTRAS TO TELL US ABOUT

  • Knowledge and experience with complex client care planning and individual/systemic advocacy
  • Training and experience in crisis intervention/de-escalation
  • Knowledge of community-based services and resources
  • Knowledge of different substances and how they may impact a person’s behaviour
  • Standard First Aid, Mental Health First Aid certification
  • French and other language skills are considered an asset in supporting the communities we serve
