The London Village offers a variety of programs to meet the diverse needs of families including a Child Care Centre, Respite Program for individuals with intellectual disabilities and Adult Day Program for seniors. Our program site sits on 9 ½ acres of beautiful green space and offers a gymnasium, pool, pavilion and playground equipment for everyone to enjoy. The Adult Day Program is a specialized day service for seniors with Alzheimer's and related forms of dementia and those with medical fragility looking for social engagement opportunities while providing a break to their family care providers. The environment is secure and homelike while offering personal supports such as: personal care & hygiene and therapeutic recreational, social, educational opportunities and special event programs.

The Adult Day Program is currently hiring a Community Nurse. This is a permanent part-time position with a rotational work schedule that will be set by the supervisor as per program hours of operation. The working hours of this position will include every other Saturdays from 9:00am to 4:00pm and one-week day shift from 10:00am to 5:00pm. All shifts will include a 30-minute hour paid meal break where applicable. Some flexibility in scheduling is required to meet program needs. This position is for someone that has a passion for and takes initiative in supporting in areas such as: personal care & hygiene, advocacy, and empowering each person we support to remain active, independent, and socially engaged.

The Community Nurse assists the Adult Day Program Supervisor and coworkers in creating and maintaining a high quality, stand-alone community program suitable for providing varying levels of care to program clients.

KEY RESPONSIBILITIES:

Clients Services:

  • Provide care in accordance with administrative policies, the College of Nurse of Ontario "Standards of Nursing Practice" and the standards of Gerontological Nursing.
  • Receive and document all medications upon arrival and ensure safe and secure storage is always maintained. Confirm all dosages and instructions for all medications and treatment prior to administration and the Medication Administration Record remains accurate. Administer and record the provision of all medications and treatments as preformed.
  • Assess and monitor the condition of participants; recognize and interpret signs/symptoms and immediately report concerns to caregivers.
  • Monitor nutritional status of participants and liaise with the caregiver, Program Supervisor, and ADP team to resolve concerns and ensure appropriate meals are ordered. Advise ADP team of any dietary restrictions or special meal related instructions; consult with FT nurse regarding weekly meal order to contracted meal service provider.
  • Provide direction, guidance and support to PSW staff and assist with distribution of refreshments/meals, assist with meal preparation and clean up as required and aid participants who require additional support during mealtime.
  • Provide foot and nail care services as scheduled by FT nurse; monitor bathing schedule for PSW's and assist with the provision of baths as required.
  • Assist with personal care routines as required and/or requested by PSW's.
  • Promote a healthy physical and psychosocial environment for all participants.
  • Perform lifts, transfers, transport and assist participants with mobility challenges.
  • Provide appropriate participant intervention when managing participant responsive behaviours and provide leadership and support to team during crisis or medical emergencies.
  • Liaise with emergency personal and caregivers during participant related medical emergencies.
  • Facilitate the safe arrival and departure of participants, maintain the client attendance sheet, and follow-up with any absences or health related concerns.
  • Communicate with transportation services regarding changes and absences.
  • Review daily Outbreak Report and provide guidance to participants/caregivers regarding attendance as required.
  • Perform end of day program closure routines.

Communication/Administration:

  • Ensure documentation accurately reflects level of personal support assistance required for each participant.
  • Maintain accurate and complete participant records of nursing observations and care/treatment and medications provided and any other pertinent information.
  • Assist in maintenance of the participant emergency file and binder.
  • Assist Program Supervisors with client file data entry as required.
  • Participate in client reassessments.
  • Report any actual or potential problems/situations to the Program Supervisor or designate.
  • Report any identified needs to team members, Program Supervisor and/or on-call designate, and caregiver.
  • Complete and submit incident reports.

Health & Safety:

  • Facilitate, lead and document monthly fire drills for the program as required.
  • Ongoing monitoring of environment for risk and safety issues and report concerns to the Program Supervisor or designate.

Human Recourse:

  • Provide support and guidance to part-time and/or casual relief staff.
  • Provide training, mentoring and orientation to new staff/volunteers/students.

Property:

  • Report any property damage, mechanical breakdown of equipment and/or repairs needed in the program.

Perform other position related duties as required.

MANAGERIAL/Technical Leadership RESPONSIBILITY:

  • This position reports directly to the Adult Day Program Supervisor or designate.
  • Uphold all Salvation Army policies, directives, and operating guidelines as contained in the Policy Manual of The Salvation Army London Village.
  • Assist the ADP Program Director, Program Supervisor and coworkers in creating and maintaining a high quality, client focused, community program suitable for providing varying levels of care to program participants.
  • Participate in program CQI initiatives.
  • Participate in the client reassessment process.
  • Provide coverage for other staff during absences.
  • Maintain up to date knowledge of Alzheimer Disease and related dementias and community resources.
  • Participate in the development of departmental policies and procedures specifically those related to medical practices.
  • Represent the Adult Day Program on internal committees as appropriate.
  • Participate in client specific and/or program specific training as required.
  • Participate in ongoing professional development and training.
  • Mandatory attendance at scheduled staff meetings.

Working Conditions:

  • This is a permanent part-time position based on 7 hours per week with the possibility of additional casual relief hours.
  • Flexibility in scheduling is required to meet program requirements; schedule including days and weekends.
  • The Supervisor will set the hours of work and work schedule.
  • Ability to lift/move up to 50 lbs.
  • Frequent lifting, twisting, bending, crouching, reaching, pushing and pulling.
  • Walking and standing for long periods.
  • Some kneeling, running may be required.

Normal hours of work: Rotational work schedule will be set by your Supervisor as per program hours of operation including every other Saturday (9:00am to 4:00pm) and one-week day shift (10:00 am to 5:00 pm) and includes a 1/2 hour paid meal break where applicable.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

  • Completed a two (2) year Community College diploma in nursing, degree preferred.
  • Current, valid registration as a Registered Practical Nurse with the College of Nurses of Ontario.
  • Current Gentle Persuasive Approach training.
  • Current First Aid & CPR Certification.
  • Current Safe Food Handling Certification.
  • Valid Foot Care certificate with recent experience.
  • U-First Training and/or P.I.E.C.E.S training would be an asset.
  • Alternative combinations of education and experience may be considered.

EXPERIENCE AND KNOWLEDGE:

  • Minimum of one (1) years of prior related nursing experience working with cognitively impaired/frail elderly populations.

SKILLS AND CAPABILITIES:

  • Bilingual in French is preferred.

  • Participate in regular supervision and performance appraisal process.

  • Proficiency in all Microsoft Office products is required - i.e. Word, Excel, Publisher, PowerPoint, Email and Internet etc.
  • May require screening through The Salvation Army Abuse Registry.
  • Ability to undergo applicable screening and background checks successfully that are satisfactory to The Salvation Army, in its sole discretion (i.e., The Salvation Army Abuse Registry) and provide an original copy of a Background Check with vulnerable sector screening.
  • Ability and willingness to develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities.
