As a member of the Advanced Practice Leadership team at SE Health reporting to the Clinical Director, Practice Transformation and Innovation, you strive for excellence in quality & scholarly professional practice. You provide expert clinical support and leadership in our operations and are instrumental in setting and upholding practice standards, advancing clinical leadership, and ensuring regulatory compliance. The Advance Practice Leader will facilitate clinical leadership capacity building, address practice-related inquiries, and develop innovative care models by leveraging research and best-practice evidence. Additionally, they will evaluate data to drive continuous process improvement while supporting education, training, and quality and risk management initiatives. With a strong focus on clinical excellence, this role plays a key part in shaping high-quality patient care.

Responsibilities include but not limited to:

· Lead the development and implementation of innovative care models to enhance patient outcomes.

· Leverage research, best-practice evidence, and emerging trends to advance clinical practice.

· Establish and uphold practice standards to ensure high-quality, patient-centered care.

· Lead program development and new models of care delivery for the target population, in collaboration with the operations leadership team and the interdisciplinary team.

· Provide expert knowledge and professional practice leadership to develop solutions/service delivery models for exceptional care.

· Foster a culture of continuous learning and professional growth.

· Monitor federal, provincial, and other relevant legislation/regulations for compliances and collaborate on strategies to address opportunities for improvement.

· Ensure compliance with regulatory and accreditation requirements.

· Develop and oversee quality improvement initiatives to enhance patient safety and outcomes.

· Implement strategies to manage risk and uphold best-practice guidelines

· Analyze and interpret clinical and operational data to inform practice improvements.

· Evaluate program effectiveness and recommend changes based on measurable outcomes.

· Collaborate with stakeholders to integrate evidence-based solutions into practice.

· Work closely with interdisciplinary teams to drive system-wide innovation.

· Engage with external partners, academic institutions, and research bodies to advance clinical excellence.

· Advocate for policy and practice changes that align with emerging healthcare trends.

· Contribute to business development initiatives as required.

· Support Clinical Practice leaders and the Clinical Practice Resource team through regular meetings, orientation, mentorship, consultation and guidance with complex clinical situations

· Provide on-call support for the Clinical Practice Resource Team and Operations teams.

Qualifications:

· Undergraduate in a healthcare-related field (e.g. RN, NP, PT, OT, etc.). Master's Degree an asset.

· Current license in good standing from a provincial regulatory body.

· 5 years of regulated health professional experience.

· Additional certifications in leadership, quality improvement, or healthcare innovation (e.g., Lean Six Sigma, PMP, Prosci) are an asset.

· Extensive clinical experience in an advanced practice or leadership role.

· Proven track record in healthcare innovation and practice transformation.

· Experience in developing and implementing new care models based on research and best practice.

· Strong background in quality improvement, risk management, and regulatory compliance.

· Knowledge of the related regulatory and legislative requirements across Western Canada provinces and territories.

· Experience in data analysis and evidence-based decision making.

· Strong leadership and communication skills to engage interdisciplinary teams and stakeholders.

· Research and analytical skills to evaluate data and integrate evidence into practice.

· Knowledge of MS Office (Word, Excel and Outlook).

· Ability to influence others and establish credibility with all levels of the organization, challenging the status quo and being a catalyst for positive change.

· Excellent organizational, planning and time management skills.

· Ability to manage multiple, often competing priorities.

· Demonstrated effective critical thinking, analytical, decision making and problem-resolution skills.

· Demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams.

This role is primarily virtual. You will be required to travel to home office in Markham, Ontario for 2 days every two months, at minimum. There may be other ad hoc travel expectations as required. A vehicle and ability to travel and some on-call is required.
