Job Description

Reporting to the Clinical Support Manager, the Clinical Assessment & Evaluation Lead will design, implement, and evaluate resident assessment and documentation strategies across Extendicare homes. The Clinical Assessment & Evaluation Lead will have in-depth knowledge and understanding of the existing RAI MDS 2.0 tool and will be responsible developing similar expertise with LTCF and using data, reports, and PointClickCare (PCC) configurations to implement best practices and standard workflows across the organization.

This position will be located at Extendicare Head Office in Markham, Ontario and will have flexibility for both remote and in-office work based in Extendicare's hybrid work model

Responsibilities:

  • Provide guidance and support to homes related to resident assessments, associated clinical and quality outcomes, and care planning protocols

  • Develop educational content and lead training sessions to support implementation of new resident assessment procedures and workflows

  • Develop build guides and configure PCC to enable standard workflows for resident assessment, care planning, documentation, and point of care practices

  • Coordinate and lead interRAI community of practices with homes and regional support teams and promote practice standards that strengthen assessment functions, validate generated assessment outcomes, support an integrated assessment and care plan model, and optimize funding

  • Act as a resource and point of contact for homes when corresponding with vendors such as PCC, CIHI and Relias for service requests, data extracts, queries, and operational reporting

  • Build and maintain professional partnerships with business partners within Extendicare to foster teamwork and cross team collaboration

Qualifications:

  • Registered Nurse or Registered Practical Nurse with a Bachelor of Science in Nursing or Diploma from an accredited nursing program

  • 3+ years of experience in an equivalent or similar role

  • In depth knowledge of PCC, Point of Care, RAI-MDS 2.0 and basic understanding of interRAI LTCF

  • Thorough knowledge of LTC legislation, program, and documentation standards

  • Demonstrated success leading new initiatives with experience in project management, coordination, and support

  • Experience with adult education strategies and facilitation

  • Proven ability to build strong relationships with individuals at all levels of an organization and external vendors and service providers

  • Ability to function effectively in ambiguity and progress projects with incomplete information

  • High degree of personal and professional integrity

  • Proficient with MS Office suite
