Background on Our Dementia Journey Journal
Our Dementia Journey Journal (ODJJ) is an interactive resource that is designed to enable sustainable relationship-building between caregivers and care providers of persons living with dementia through the sharing of meaningful and relevant information pertaining to physical, functional and social care and support. It also aims to help caregivers and care providers (re)negotiate their respective roles as the circle and/or setting of care changes along the dementia journey and optimize their respective knowledge of and about the persons living with dementia PLWD). The evidence-informed ODJJ is available on analog, digital and mobile platforms and is available in English, French, Hindi, and Punjabi. From spring 2024 - fall 2024, the ODJJ has been implemented and evaluated in LTC homes, First Nation communities and South Asian communities. In the next phase of the project, the team is looking to implement the ODJJ in additional communities across Canada.
We are currently seeking a passionate and dynamic Temporary Full Time Community Engagement Lead to support the implementation and rollout of the Our Dementia Journey Journal (ODJJ) across various organizations such as Alzheimer's Societies, adult day programs, long-term care homes, retirement homes, and other community settings across Canada. In this role, the successful candidate will build and nurture relationships with key community partners in these organizations to introduce and facilitate the adoption of the ODJJ resource. They will be tasked with executing an implementation plan with marketing materials and supporting the resource's integration into diverse community settings. This is a 4-month contract position.
