Job Overview:

We are seeking a highly motivated and experienced Virtual Clinical Care Manager to lead our Virtual Nursing Team. This team of registered nurse clinical experts plays a critical role in supporting clinicians and patients through four key domains of practice: Just-in-Time Clinical Support, Complex Care Planning, Telepractice/Virtual Care Delivery, and Practice Leadership. The ideal candidate will possess strong leadership skills, clinical expertise, and a passion for innovative healthcare delivery. This role will involve a balanced split between managerial duties and active participation in team responsibilities, including managing calls and other role functions.

Key Responsibilities:

1. Just-in-Time Clinical Decision Support:

  • Oversee the provision of real-time clinical support to clinicians and patients.
  • Ensure timely and accurate responses to clinical inquiries.
  • Monitor and evaluate the effectiveness of support provided, implementing improvements as needed.

2. Complex Care Planning:

  • Support the team in developing and implementing comprehensive care plans for patients with complex needs.
  • Ensure proper timelines for responses and escalate cases to appropriate organizational leaders for consultation as required.
  • Collaborate with multidisciplinary teams to ensure holistic patient care.

3. Telepractice/Virtual Care Delivery:

  • Manage the delivery of telehealth services, ensuring high-quality, reliable virtual care.
  • Develop and implement innovative telepractice protocols to meet evolving business needs.
  • Monitor telepractice performance metrics and implement improvements to enhance service delivery.

4. Practice Leadership:

  • Provide leadership and mentorship to the virtual nursing team.
  • Foster a collaborative and supportive team environment.
  • Lead initiatives to improve clinical practice and patient care outcomes.
  • Ensure compliance with all regulatory and accreditation standards.

Accountabilities:

The Virtual Clinical Care Manager is responsible for overseeing the scheduling of team members to ensure adequate coverage and timely support, conducting regular performance reviews and providing constructive feedback, supporting the continued growth and development of all team members in alignment with the Professional Practice mandate. The role manages the day-to-day operations of the virtual nursing team to ensure efficiency and effectiveness, and developing and implementing strategies to scale the team in alignment with the organization's strategic plan and business priorities.

Skills and Competencies:

The ideal candidate will possess strong leadership and team management abilities, deep clinical expertise particularly in gerontology, chronic disease management, wound care, palliative care and complex care planning. They will have excellent verbal and written communication skills to effectively interact with clinicians, patients, and team members, strong analytical and critical thinking skills to address clinical and operational challenges, proficiency with telehealth platforms, EHR systems, and other healthcare technologies, and the ability to thrive in a fast-paced, dynamic environment and manage multiple priorities.

Qualifications:

  • Registered Nurse (RN) with an active license.
  • Master's degree in nursing; will consider a candidate in last 25% of program
  • Minimum of 5 years of recent clinical nursing experience, with at least 2 years in a leadership role.
  • Demonstrated success in supporting an organization to achieve its goals and objectives
  • Experience in virtual healthcare delivery and telehealth.
  • Possesses an innovation and growth mindset
  • Strong knowledge of care planning and management of complex patient cases.
  • Excellent communication, leadership, problem solving and organizational skills.
  • Ability to work effectively in a remote environment and manage a virtual team.
  • Proficiency with healthcare technology and electronic health records (EHR) and MS Office applications.

Hours:

  • Monday to Friday, 9 AM to 5 PM. Candidate may be required to flex schedule to meet operational needs on evenings, weekends, and stat holidays
  • Leadership on-call coverage as required
