As a Care Connector (Status: Temporary, Full-Time), helps patients navigate through a complex health care system by connecting people to an appropriate primary care practitioner. Reporting to the Manager, Patient Services, the Care Connector supports the Health Care Connect Program in providing patients with assistance navigating through a complex health care system. The Care Connector works collaboratively with patients and others in their support network to gather information to make a successful connection with an appropriate primary care provider based on the patient’s care needs. The incumbent will maintain contact with patients on the Ontario Health atHome priority list to keep them informed about potential connections to a primary care provider. The Care Connector works closely with physicians and nurse practitioners to establish and maintain effective relationships in order to build and maintain an inventory of family health care professionals willing and able to accept new patients. What must you have?
Registered Practical Nurse (RPN) or Registered Nurse (RN) with current registration with the College of Nurses.
Good knowledge of primary care services and other community resources with Ontario Health atHome, as well as the roles of health care professionals.
Understanding of issues and priorities within the health care sector.
In depth understanding of the Ontario Health atHome business, related priorities and requirements for Patient Services as well as understanding of the relevant legislation.
Effective planning, organizing and evaluation skills to manage multiple patients provide information reports and take corrective action as required.
Ability to use MS Office applications.
Strong interpersonal, communication and collaboration skills.
Effective listening and strong facilitation skills to deliver information effectively in a variety of settings including one-on-one, team meetings, and presentations.
Ability to engage in regular and frequent travel to meet with community groups, primary health care professionals and other health and social services is required.
Regular attendance at work is required.
