What would my role be?

Reporting to the Manager, Admissions and Patient Flow, you will:

  • Facilitate pre-admission, triage, and coordinate admissions of patients to CCC, Rehab, and Palliative Care. While responsibilities are divided between two individuals (e.g., rehab/palliative and CCC), you must collaborate closely for an integrated approach and cover for each other as needed.
  • Maintain effective and efficient processes to facilitate the timely triage and admission of eligible patients, working closely with internal and external partners.

How do I qualify?

Required:

  • An Undergraduate degree in Nursing or Allied Health from an accredited university.
  • Minimum five (5) years of experience in a hospital setting, specifically in complex continuing care (CCC), rehabilitation (Rehab), and/or palliative care.
  • Valid professional certification in a healthcare discipline, which could include credentials as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or other allied health professions.

      Preferred:

      • Fluent in French and English (Candidates who do not meet this language requirement are eligible to apply for this position and may be considered if there are no other suitable applicants who meet this requirement.)

      What skills should I possess to succeed in the role?

      • Advanced clinical assessment skills and clinical judgment for determining patient eligibility and planning admissions.
      • Strong communication skills and the ability to interact with multiple agencies and individuals.
      • Strong organizational skills and leadership abilities.
      • Knowledge of the healthcare system.
      • Demonstrated ability to function in a self-directed manner.
      • Able to balance competing priorities in a fast-paced environment, aligning with patient flow needs and bed availability.
      • Advanced computer literacy for efficient frequent navigation of multiple different computer applications, including electronic patient record systems.
      • Advanced Knowledge of Medical Terminology.
