What would my role be?

Reporting to the Manager of Outpatient Programs, Bruyere Health and the Program Manager, Regional Geriatric and Community Intervention Program, you will:

  • Provide an in-home multidimensional comprehensive geriatric screening assessment to referred clients, to determine a frail older person's biomedical, psychosocial, functional and environmental needs, and initiate early interventions in the form of recommendations that aim to optimize the client's health, function and safety.
  • Be responsible for collecting data in the context of clinical assessment, and for the purpose of program evaluation.
  • Be involved with the client, the caregiver, and community resources in order to promote clients' functional independence in the community.

How do I qualify?

Required:

  • Bachelor's degree of Science in Nursing.
  • Minimum of three (3) years of clinical geriatric experience in a community setting.
  • Valid certificate of registration from the College of Nurses of Ontario (CNO).
  • Valid driver's license and access to a vehicle.
  • Fluent in French and English (Candidates who do not meet this language requirement are eligible to apply for this position and may be considered if there are no other suitable applicants who meet this requirement).

  • Superior skills in documenting the assessment, analysis and interventions in a clear, concise report format
  • Able to work autonomously in the community
  • Able to adapt and be flexible to a variety of client's living environments
  • Excellent critical analysis skills
  • Able to screen, assess and identify risk within identified domains of the Comprehensive Geriatric Assessment
  • Able to analyze and interpret assessment findings to formulate a cohesive clinical impression
  • Able to provide interventions focused on optimization of function, safety and quality of life
  • Demonstrates strong teamwork skills such as interpersonal communication and collaborative decision making
  • Fundamental understanding of physiological and biopsychosocial mechanisms of the aging processes, age related changes to functioning and the impact of frailty
  • Knowledge of currently accepted recommendations for primary and secondary prevention of common geriatric syndromes
  • Knowledge of community resources available for seniors
  • Knowledge of the regional health care system in the area of geriatrics
  • Core geriatric knowledge of the geriatric syndromes
  • Proficiency in using Electronic Medical Record and Microsoft Home
  • Advanced computer skills
  • Advanced knowledge of medical terminology
