Position Description

The Nurse Practitioner provides a wide range of services including comprehensive triage, physical assessment, diagnosis and treatment as well as counseling, health promotion, and preventative care to clients with varied health, social, cultural and economic needs. The successful candidate will work to full scope of practice within our Walk-in Clinic, with the ability to function independently and as a member of an interprofessional team in a culturally sensitive manner. The successful applicant will also be required to provide outreach health care services and support an ODSP clinic.

Requirements

Education

  • Current extended class certificate (NP-PHC) and unrestricted registration with the College of Nurses of Ontario Registration in the Extended Class with the College of Nurses of Ontario.
  • Current membership with RNAO or NPAO, including professional liability coverage.
  • Certification in CPR (BLS-C)
  • Knowledge of ODSP application process.
  • Previous Hepatitis/HIV treatment experience preferred.

Professional Experience

  • Five years’ experience working within a collaborative interdisciplinary team with two years in a community or primary care setting.
  • Strong clinical skills and diagnostic abilities for clients across the life span
  • Previous experience working with culturally diverse and marginalized populations.
  • Interest in providing outreach services.

Knowledge, Skills and Abilities

  • Knowledge of primary health care and the social determinants of health
  • Knowledge of concepts of health promotion, disease prevention, behavior change counselling, program planning, individual and group counselling
  • Knowledge and awareness related to cultural competence.
  • Demonstrated understanding of and commitment to the principles of harm reduction
  • Comfort with medical directorship and delegation
  • In-depth knowledge of health assessment and interventions in the family practice context
  • Knowledge of non-violent crisis intervention
  • Ability to work independently and be self-directed.
  • Strong understanding of professional practice obligations and seek consultation with regulatory college as required to support sound and ethical clinical practice.
  • Excellent communication skills and ability to adapt communication styles to meet the needs of clients, health team members and community partners.
  • Ability to work under pressure both independently and as part of a team
  • Outreach requirement, unrestricted Class G driver’s license and access to efficient, reliable transportation.
  • Successful completion of Police record Check for vulnerable populations.

Language Capability

  • English is required.
  • French or other language(s) are an asset.
