What would my role be?

Reporting to the Director, Pharmacy and under the supervision of Pharmacy Leadership, you will:

  • Conduct individual patient assessments and provide recommendations and follow ups on medication usage.
  • Provide patient education regarding medication use.
  • Collaborate with the interprofessional team to improve the medication management program for better patient care.

How do I qualify?

Required:

  • Undergraduate degree in Pharmacy
  • Minimum of one (1) year experience as a Pharmacist in a patient oriented environment
  • Registration as Pharmacist with the Ontario College of Pharmacists
  • Hospital pharmacy residency or equivalent clinical experience

Preferred:

  • Fluent in French and English (Candidates who do not meet this language requirement are eligible to apply for this position and may be considered if there are no other suitable applicants who meet this requirement.)

What skills should I possess to succeed in the role?

  • Demonstrated expertise in practicing Pharmaceutical Care, including activities such as conducting medication histories, identifying medication-related problems, generating care plans, monitoring care plans, documenting in the medical record and communicating and collaborating with the inter-professional care team as required
  • Advanced interpersonal, organizational and communications skills
  • Able to work in an inter-professional team environment
  • Able to prioritize, manage time effectively and be flexible in a very active work environment
  • Intermediate computer skills
  • Advanced knowledge of medical terminology
