Job Opportunity
The Practice Facilitator works closely with the Team Lead for the Community Response Team (CRT) and NESI Program at the Somerset West Community Health Centre. The Practice Facilitator supports the Community Response Team, comprised of Community Workers (CWs), as they provide community response services in a designated area. Although this support provides opportunities to reflect and learn, it also involves coaching and working alongside Community workers.
Position Description
The Practice Facilitator will be responsible for supporting the Community Response Team (CRT) at Somerset West Community Health Centre through ongoing training and regular reflective practice sessions focused on topics such as boundaries and self-care and workplace norms. In addition, they will also be responsible for working alongside CRT delivering outreach during evening and weekend hours. This will include providing real-time support and development to CRT in providing service delivery.
