Ontario Health atHome is seeking an Clinical Lead, South West Regional Wound Care Program
Opportunity Summary:
The South West Regional Wound Care Program (SWRWCP) advocates for the integrated seamless, timely and equitable delivery of high quality, safe, efficient, and cost effective, person-centered, evidence-informed skin and wound care to the people of Ontario Health atHome, regardless of the setting. The SWRWCP is hosted by Ontario Health atHome South West region, but supports all hospitals, long term care (LTC) homes, contracted community nursing service provider agencies and Primary Care Providers. The SWRWCP advocates for Ontario Health atHome health system partners to become leaders in integrated skin and wound care management. The vision of the SWRWCP is: "Integrated, evidence-informed skin and wound care - every person, every health care sector, every day."

As a member of the Quality team, the Clinical Lead for the SWRWCP ensures integrated delivery of wound care that spans the continuum of care within Ontario Health atHome South West. In collaboration with health system partners, the Clinical Lead formulates and executes a coordinated strategy for the delivery of wound care based on leading practice. This is a temporary full time, two (2) month opportunity open to any of our South West office locations (London, St Thomas, Stratford, Woodstock or Owen Sound).
While some elements of the role are currently being performed remotely, the successful incumbent will be required to regularly report to their assigned office location.
