Opportunity Summary:

The South West Regional Wound Care Program (SWRWCP) advocates for the integrated seamless, timely and equitable delivery of high quality, safe, efficient, and cost effective, person-centered, evidence-informed skin and wound care to the people of Ontario Health atHome, regardless of the setting. The SWRWCP is hosted by Ontario Health atHome South West region, but supports all hospitals, long term care (LTC) homes, contracted community nursing service provider agencies and Primary Care Providers. The SWRWCP advocates for Ontario Health atHome health system partners to become leaders in integrated skin and wound care management. As a member of the Quality team, the Clinical Lead for the SWRWCP ensures integrated delivery of wound care that spans the continuum of care within Ontario Health atHome South West. In collaboration with health system partners, the Clinical Lead formulates and executes a coordinated strategy for the delivery of wound care based on leading practice. This is a temporary full time, two (2) month opportunity open to any of our South West office locations (London, St Thomas, Stratford, Woodstock or Owen Sound).

While some elements of the role are currently being performed remotely, the successful incumbent will be required to regularly report to their assigned office location.

What will you do?

• Oversee and promotes the ongoing development and maintenance of a sustainable knowledge translation framework to support all care providers across all health care settings.
• Promotes integration, coordination and service innovation in developing strategic and change management plans, and implementation work plans for program staff.
• Ensures the development and implementation of cross-organizational wound care education and resources including evidence informed guidance and tools, expert resources and education events, expert wound care specialists and Wound Care Champions to enhance health service provider quality and skill improvement capacity to advance skin and wound care management and improve clinical outcomes. Identifies initiative-specific educational needs of partners and staff, and provides learning opportunities to promote knowledge to action.
• Meets with senior leaders to establish program objectives and ensures projects meet objectives, are timely and cost efficient, patient focused and align with the vision of the SWRWCP and Ontario Health atHome Integrated Health Service Plan (IHSP)
• Promotes the ongoing alignment of wound product selection through Ontario Health atHome
• Supports the development and delivery of annual work plans based on priorities set by Ontario Health atHome focused on, but not limited to, effective prevention and management of wound care across the system of care.
• Participates on the Ontario Health atHome multidisciplinary planning tables and committees, representing SWRWCP as it relates to the IHSP and system improvements. Works collaboratively with the Ontario Health atHome to address program needs and achieve targets
• Maintains a cross sector partnership governance structure (SWRWCP Strategic Steering Committee) and reporting processes to support the SWRWCP and promote collaboration and investment in improving and integrating wound care.
• Establishes and maintains strong relationships and partnerships with South West health service providers across four health care sectors and 100+ organizations (primary, acute, long term care and community) as well as other regional and provincial and national key stakeholders with respect to program deliverables, planning and opportunities for collaboration. Establishes and identifies capacity and program plans in conjunction with system leaders.
• Undertakes strategic and operational dialogue with Ontario Health atHome representatives, primary, acute, long term care and community partners/stakeholders to determine current
• Develops, monitors and reports on SWRWCP activities to ensure achievement of program objectives/deliverables.
• Informs best practice approaches to change initiatives through the application of various quantitative and qualitative performance measures. Continuous quality improvement through measurement and evaluation of cross sector wound care outcome indicators in collaboration with health system partners in Ontario Health atHome
• Monitor performance of quality improvement initiatives the SWRWCP supports.
• Supports the Program Lead in the development of a high performing clinical team for the delivery of the SWRWCP.
• Develops and implements procedures for funding proposals including the development of business proposals, creating the budget, review of authorization forms, educating staff on processes and tracking of funds
• Determines most efficient formats, charts and tables for thorough and accurate reporting of funds and program initiatives
• Evaluate training and development needs, design and deliver education programs and resources for those front line service providers/clinicians/workers that provide a basic level of wound care to the homeless and under-housed; and,
• Keep abreast with best practices and on the latest tools around skin and wound care education.
• Develop and educational framework from novice to proficient levels using a curriculum framework using a consensus group to define educational standards for wound care across multiple disciplines.
• Lead the development of quality improvement opportunities to support improved performance against the SWRWCP process and cross-sector clinical indicators;
• Lead the development of a centralized interdisciplinary wound consultation service pilot within Ontario Health atHome; and,
• Develop and lead the delivery of other priority for investment sub-projects as approved by Ontario Health atHome
• Initiate and participate in research projects proposed by Ontario Health atHome stakeholders to improve patient wound care outcomes across all sectors of health care
• Provide clinical expertise and input for the strategic development of Ontario Health atHome programs and initiatives

What must you have?

• Bachelor's degree in any registered healthcare discipline (e.g. nursing, physiotherapy, occupational therapy, etc.) Required
• Masters level education in Wound Healing and Tissue Repair (Masters of Clinical Science (MClSc), Master of Science (MSc) in Wound Healing and Tissue Repair or equivalent) Required
• 3 to 5 years of demonstrated leadership in wound care management experience with the ability to manage complex projects involving multiple agencies and stakeholders across the continuum of care.
• Experience developing, evaluating, implementing and promoting initiatives aimed at improving integration within the health care system across multiple sectors.
• Experience leading medium to large-scale projects including developing the appropriate frameworks and accountability mechanisms.
• Experience working effectively in partnership with broad range of individuals and organizations of diverse backgrounds, preferably within the health care sector.
• A proven track record of effective working relationships with internal and external customers (inclusive of Primary Care Physicians) and clients and the ability to build teams and lead through system level changes.
• An ability to problem solve, resolve conflicts and achieve outcomes.
• Strong collaboration skills to participate on projects and committees with colleagues across Ontario Health atHome and/or wound care service providers.
• Ability to deliver information effectively and interpret data clearly in a variety of settings including one-on-one with staff and in team meetings.
• Ability to deal constructively with a variety of contentious/difficult situations, discuss sensitive information and influence/persuade others to follow a recommended course of action.
• Exceptional written/oral communication and presentation skills. Must be able to communicate effectively with stakeholders at all levels and to influence change, provide instructions and guidance to staff with respect to activities, challenges and questions. Must be able to explain findings, recommendations and innovations. Excellent interpersonal, diplomacy and presentation skills.
• Ability to influence decision making through consultation and collaboration when working with internal and external stakeholders.
• Excellent group facilitation and process mapping skills (using methodologies such as: Lean, Institute for Healthcare Improvement) to draw out insights from participants, engage them in a discussion, and enable them to reach agreements that influences change in practice and integration.
• Ability to interpret and utilize data for quality improvement.
• Knowledge of Ontario's health care system, its stakeholders, programs and issues.
• Knowledge of primary care service delivery models in Ontario.
