Responsible as the first point of contact, for both the Adult Day and Caregiver support Programs and Services, the incumbent will provide information and guidance to individuals inquiring about either program, complete the initial client/caregiver intake and subsequent assessments, facilitate and monitor all aspects of client and/or caregiver admissions, and discharges in either program. The incumbent will also take an active, shared role in programs and services offered through the Scotiabank Learning Centre. All duties are performed in accordance with the Mission and Values of Providence Healthcare.
MAJOR RESPONSIBILITES:
Responds to all information requests through telephone, mail, and email, for prospective clients and their families;
Guides families to appropriate services based on their needs and/or makes appropriate referrals on behalf of the family;
Arranges and provides all aspects of the client intake process; initiates assessments of potential clients/caregivers, offers initial services with the families in collaboration with other staff, and monitors the waiting list for all services in conjunction with families and staff;
In conjunction with Adult Day Program manager, operations leader, and colleagues, organizes, facilitates and or coordinates all aspects of the Learning Centre services such as support groups, education sessions and resources;
Ensures the safe, welcoming and clean appearance of the Scotiabank Learning Centre;
Maintains current knowledge of social media/ communication platforms, resources (electronic and paper), blogs, and websites;
Researches new and innovative programs, services, technologies and tools that support individuals in our community who are caring for individuals with life altering events;
Creates and develops positive relationships, ongoing communications and promotion of the Adult Day and Caregiver support Program services to referring agencies and community partners;
Participates in the delivery of presentations and participation in community events as appropriate;
Markets the services of the Adult Day and Caregiver support Programs;
In conjunction with operational leadership maintains partnerships with organizations offering community based services;
Works with the Administrative Assistant, Adult Day Program, to verify, update and maintain that all client information required for care.
Tracking and entering data for SLC reporting requirements.
QUALIFICATIONS/SKILLS REQUIRED:
Degree or diploma in Activation, Recreation, Nursing (RN, RPN level), Social Service Worker or Gerontology or an undergraduate degree in a health-related field;
Advanced knowledge of community based services and resources for individuals with dementia and their families;
A minimum of 3-5 years experience working in a community based setting with knowledge of associated terminology;
Knowledge of client data base software systems such as HiNext TREAT;
Must have experience administering the inter RAI Community Health Assessment (interRAI CHA) and Functional and Mental Health Supplements;
Experience with Resource Matching and Referral pathways;
Experience with the Integrated Assessment Record;
Experience and understanding of the needs of adult and geriatric populations;
Ability to work with elderly with dementia;
Well developed presentation, networking and marketing skills;
Ability to lead and facilitate group sessions;
Demonstrated patience for both telephone and person-to-person situations;
Consistently demonstrate sensitivity and compassion to client and caregiver concerns and fears;
Strong organizational skills, problem-solving skills and critical thinking skills;
Excellent oral, written and interpersonal and communication skills;
Sound judgment and diplomacy are key competencies;
Computer literacy with developed skills in various software applications including word processing skills (i.e. Windows 2000 and MS Office), spreadsheets, databases, and presentation/graphics;
Demonstrated ability to manage high volume and varied work assignments with accuracy and flexibility and in a timely fashion;
Ability to work independently under pressure and establish priorities under time constraints;
Flexible and ability to take initiative in responding to needs as they arise;
Knowledge and sensitivity to work in a culturally diverse environment;
Self-motivated with demonstrated ability to function as a part of a team and/or independently, creative, and exhibits a good sense of humour;
