POSITION SUMMARY:

To provide psychosocial assessment and social work services to patients/clients and their families. All duties are performed in accordance with Providence Healthcare's Mission and Values.

MAJOR RESPONSIBILITIES:

  • Provides psychosocial assessment and direct social work services in accordance with Social Work Clinical practice protocols/standards to patients/clients and their families as well as develop and facilitate appropriate intervention plans utilizing a variety of treatment modalities;
  • Participates in interdisciplinary patient/client care, planning, along the continuum of admission and discharge;
  • Provides professional support, education and training to other team members in their work with patients/clients and their families;
  • Participates with the Professional Practice Leader, Social Work and peers in professional practice planning and implementation and evaluation in a manner, which ensures innovative, creative and current professional practice in meeting patient/client needs;
  • Ensures professional development through reading current literature, attending relevant workshops and conferences, participation in in-services and social work professional development activities;
  • Participates in the development and implementation of educational initiatives for clients, families, staff, volunteers, students and relevant community organizations;
  • Promotes the development of Social Work and/or practice internally and externally through research, evaluation studies, and other professional activities e.g. publication.

QUALIFICATIONS/SKILLS REQUIRED:

  • Master of Social Work degree from an accredited school of social work and eligibility for affiliation with professional associations (Ontario Association of Professional Social Workers and the Ontario College of Certified Social Workers);
  • Knowledge of and experience in social work practice in a health care environment preferably with expertise in working with adults and their families and knowledge of the associated problems of aging and institutionalization;
  • Ability to execute early problem and need identification through professional assessments and skillful implementation of all planned social work interventions in cooperation with the patient/client, his/her family, interdisciplinary team and community resources;
  • Ability to work cooperatively within an interdisciplinary team, particularly in a coordinative role;
  • Comprehensive knowledge of and ability to mobilize community resources;
  • Ability to work with individuals, families and groups;
  • Interest and experience in staff education and program development;
  • Ability to communicate clearly and accurately in verbal and written reports;
  • Demonstrated motivation for ongoing professional development through continues education. This is normally acquired through several years of social work experience with proven ability in clinical practice;
  • Very good attendance record;
  • Good performance record.
