Job Description

SOCIAL WORKER- Extendicare Scarborough

Candidates are invited to apply for the position as a Social Worker, located at our 151-bed Extendicare Scarborough Home in Scarborough, Ontario.

Reporting to the Administrator, the Social Worker works in collaboration with an interdisciplinary team to advocate and provide support and services to residents, families and staff.

RESPONSIBILITIES
• Completes resident pre-admission, admission and ongoing social work assessment to identify goals and strategies, expected outcomes for care and target dates.
• Participates in interdisciplinary team conferences as required
• Acts as liaison, as appropriate, with families.
• Provides counselling or group sessions for residents and families as identified - i.e., orientation adjustment programs, crisis problem solving, bereavement counselling, etc.
• Acts as a resource person to staff who have identified needs for counselling, referrals or support.
• Maintains established department plans, policies, procedures.
• Plans, provides or arranges for in-service programs for facility staff, residents and families as identified.
• Encourages and supports professional growth and development of staff.
• Maintains all required records, reports, statistics, etc. for both residents and department in accordance with policies, procedures and legislative requirement.
• Participates on committees related to resident care and operation of the facility as required.
• Establishes and maintains communications with other disciplines, residents and families, to ensure resident needs are being met.
• Liaises and consults with government officials, company resources and professionals in relation to resident care needs.
• Other duties as assigned by supervisor.

QUALIFICATIONS
• Bachelor's degree (BSW) in Social Work from an accredited University.
• Master's degree (MSW) in Social Work from an accredited University preferred
• Sound knowledge of community resources and services
• Sound knowledge in the fields of aging, gerontology and dementia
• Previous experience in long term care preferred.
• Knowledge of applicable legislation, individual counselling and group sessions.
• Knowledge of occupational health and safety practices, principles and legislation.
• Ability to read and write English as well as understand complex verbal and written instructions in English.
• Bilingualism (English & French) an asset
