Wage Range: $39.17 - 48.96 per hour (to commensurate with experience and consistent with UHN compensation policy)
Hours: 37.5 Hours per Week
Status: Temporary Full-Time (1 Year Contract)

Job Summary

The Pediatric Oncology AfterCare Clinic at Princess Margaret Cancer Centre (PMH) is seeking a Clinical Program Coordinator - Registered Nurse to join their dynamic team focused on delivering high quality care to survivors of childhood cancer. In this role you will provide patient-centered nursing care to mostly young adults in an exclusively outpatient setting. You will work in collaboration with a team of expert interdisciplinary healthcare providers to organize and coordinate all aspects of care for patients in the PMH AfterCare Clinic. An additional focus in this role is helping patients and families transition their survivorship care to PMH from SickKids and other Pediatric Oncology centers. This position allows for a flexible, in-hospital and work-from-home model.

Duties

  • Collaborate with the interdisciplinary AfterCare Team to support childhood cancer survivors
  • Attend the weekly clinic on Monday mornings, meet with all patients by rooming them and performing vital signs
  • Conduct detailed chart abstraction for new referrals with ongoing maintenance and preparation for weekly clinics
  • Triage patient phone calls, managing clinical concerns, providing psychosocial support and education
  • Help patients/families transition their survivorship care from SickKids to PMH, which includes participation in the planning and execution of the AfterCare Transition Workshop 2-3 times per year
  • Partner with the AfterCare RN Clinic Program Coordinator at SickKids to streamline the transition process including reviewing all new referrals
  • Act as a resource and point of contact for community partners (eg. primary care providers) for Pediatric Oncology AfterCare
  • Participate in AfterCare research, coordinate with research teams at SickKids, PMH, and other centers

Qualifications

  • Registered Nurse with a Baccalaureate degree and/or Diploma in Nursing
  • Registered with the College of Nurses of Ontario
  • Minimum 5 years of paediatric oncology nursing experience, outpatient oncology nursing experience is preferred
  • Excellent organization and solution-oriented problem-solving skills with an ability to manage many priorities related to the care of a large patient population
  • Demonstrated ability to utilize your critical thinking and teamwork abilities
  • Excellent interpersonal and communication skills
  • Commitment to continuous learning and professional development
