Hours: 37.5 hours per week
Salary: $44.51 to $55.64 per hour (To commensurate with experience and consistent with UHN compensation policy)
Shifts: Monday-Friday, 8 am-4 pm
Status: Temporary Full-Time (6 months)

Position Summary

As an integral member of the Nursing Team, the responsibilities of the CLINICAL COORDINATOR, include: utilizing and applying current specialized knowledge and expertise on recent advances in nursing practice to facilitate the effective management of clinical activities and the coordination of in-patient care and continuity in patient care management; performing patient care responsibilities; performing teaching/education responsibility; participating actively in Quality Improvement initiatives; participating in academic and research activities/projects; performing administrative responsibilities; performing cross-functional and/or other duties consistent with the job classification, as assigned or requested

The Clinical Coordinator supports the program medical and clinical leadership team to ensure effective management, planning and coordination of patients' care trajectories and needs. This role will involve clinical support on 9A Orthopaedics inpatient unit and supporting patient experience and navigation.

Duties

Clinical Support and Coordination:

Utilizes and applies current specialized knowledge and expertise on recent advances in nursing practice to coordinate and facilitate the effective management of clinical activities and the coordination of inpatient care and continuity in patient care management.

  • Participates in the development, implementation and evaluation of systems, which support the efficient and effective operation of the clinic/unit including establishing administrative and technical systems to effectively measure the utilization of resources and monitor patient flow; by ensuring appropriate methods of data collection/statistical analysis, proper records management/documentation procedures, developing appropriate schedules, etc.
  • Investigates and resolves problematic situation in the unit, including: patient and/or staff clinical and technical problematic situations, recommending alternative courses of action to resolve problems, within scope of authority.

Performs patient care responsibilities:

  • Collaborates with members of the Team to plan the care of patients, arranging appropriate communication.
  • Acts as an expert resource and clinical role model to members of the Team, patients/clients and community practitioners.
  • Acts as an advocate for patients and families.
  • Collaborates with the care team to deliver and effectively communicate all current care issues.
  • Utilizes theory and research as a basis for activities within the team.
  • Utilizes and applies current specialized knowledge to counsel patients/clients in area of expertise.
  • Assess and triages (24/7 Ortho telephone line) to determine quality change management of patient care needs.
  • Perform teaching/education responsibility.
  • Document appropriately conversations that occur with patients/families.
  • Perform cross-functional and/or other duties consistent with the job classification, as assigned or requested.

Participates actively in Quality Improvement initiatives.

  • Participates in the evaluation of patient and teaching outcomes.
  • Summarizes and evaluates findings related to the quality management program.
  • Collaborates in the development and review of policies, procedures and protocols.
  • Performs appropriate audits for refinement and optimization of unit function.
  • Monitors and reports problems with products/equipment.
  • Monitors and evaluates the care delivered, including: reviewing patient charts/reports and/or liaising with patients to facilitate addressing patient/family member(s) needs, to ensure nursing care is carried out as directed by the treatment plan and/or appropriate communication is delivered.
  • Recommends and facilitates the implementation of change management to improve patient care, as required.
  • Identifies patterns and trends in care delivery and develops alternative modified approaches.
  • Implement best practices and quality improvement initiatives to enhance patient care and satisfaction.

Participates in academic and research activities/projects

  • Utilizes theory and research as a basis to set patient care standards and plan patient care.
  • Identifies researchable clinical problems and assists in the design of research projects.
  • Facilitates the utilization of evidence based research practice to provide care for patients.
  • Disseminates research findings through presentations and publications.
  • Participate in academic and research activities/projects, as appropriate.

Performs administrative responsibilities

  • Participates in committees relevant to the program.
  • Facilitates the development of specific strategies.
  • Maintains applicable statistics.
  • Liaises with appropriate organizations, corporate leaders, as required to promote the clinic's/department's activities.

Data Strategy and Analysis:

  • Conduct data analysis to derive insights into clinic performance, patient outcomes, and operational efficiency.
  • Support the preparation of data reporting.
  • Serve as a point person for inquiries and concerns related to questions and follow-up coordination.

Qualifications

  • Bachelors of Science in Nursing, Master's Degree preferred
  • Current certificate of competency (licensure) from the College of Nurses of Ontario
  • Completion of Basic Cardiac Life Support (B.C.L.S.) program
  • Up to 4 (Four) years current and relevant experience, including experience working in an advanced clinical practice role or clinical coordination preferred
  • Effective organizational, interpersonal and communication skills
  • Excellent decision-making and problem solving skills
  • Excellent time management skills
  • Excellent teaching skills with the ability to teach one-on-one and/or groups
  • Ability to function collaboratively within a multi-disciplinary team and/or independently
  • Ability to work in a self-directed manner
  • Demonstrated awareness of and adherence to professional and ethical standards
  • Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
  • Computer literacy
  • Research experience preferred
  • Membership with the RNAO, CONA, NAON preferred
