THE POSITION

The Clinical Resource Leader coordinates daily activities to ensure optimal client care and experience, by leading an interdisciplinary team that models a holistic strengths-based approach to care, utilizes a LEADS framework, employs best practices and delivers value based patient centred, and cost effective services. Effective communication and collaboration with families are core expectations of this role. The clinical resource leader also manages the nursing requirements for the Brain Injury Rehabilitation Team (BIRT) including nursing replacement, scheduling and workload analysis

This position reports to the operation manager to ensure the program goals and objectives are met in alignment with the strategic plan.

KEY RESPONSIBILITIES

  • Provides leadership to the inpatient interdisciplinary team, ensuring coordination of care activities including team meetings (e.g. facilitating bedside rounding and team rounds)
  • Ensures family experience is optimized by meeting regularly with family members, collaborating proactively to find solutions to concerns raised by families
  • Facilitate and/or participate in ongoing communication with family to mediate and resolve issues and concerns with respect and compassion, in collaboration with interdisciplinary team for client/family satisfaction.
  • Facilitate a smooth transition from the referring centre to Holland Bloorview by working collaboratively with the Family Resource facilitator
  • Teach direct care providers how to assist clients/families to be health literate and become managers of their own care.
  • Collaborate with the operations manager to identify gaps in processes, policies and procedures and support the sustainable delivery of excellent client-centred care.
  • On a daily basis, determine weekly nursing staffing needs and provide direction to the centralized schedule ensuring continuity of care, equity of nursing workload, appropriate staffing mix, and quality of nursing work life. This is done in collaboration with other Clinical Resource Leaders considering budgeted FTE, client's status/acuity and strengths, client clinic appointments, recreation outings and categories of staff, vacation/illness/LOAs, experience and workload.
  • Revise master schedule to incorporate new hires, long term disability, approved leave of absence, transfers, maternity/paternity leaves and secondments in collaboration with Central Scheduler and Operations Manager to ensure unit coverage and client safety.
  • Reconcile and maintain accurate and organized accounts payable records to provide user departments with up-to-date expenditure information for budget monitoring and forecasting purposes.
  • Work in collaboration with the Operations Manager in the analysis and future planning of budgeted resources, variances and utilization to ensure cost effective resources
  • Collaborate with Nursing Education Coordinator by taking part in assessing, planning, implementing and evaluating the learning needs of nursing staff to develop team's professional and teaching skills., and capacity-building.
  • Review New Intakes and ensure special clinical needs are addressed prior to admission such as appropriate resources, up-to-date critical information and specialized equipment.
  • Work in collaboration with the operations manager, shares accountability for achieving program targets, managing risk, improving quality and safety and advancing patient care outcomes.
  • Participates in Risk Management Identification to ensure a safe working environment.
  • Coach, mentor, support, and facilitate the creation and maintenance of an environment that fosters creativity, and professional judgment.

  • Builds employees motivation, confidence and competence through performance feedback
  • Provide support to the operations manager in the completion of performance evaluations and to uncover the hidden strengths of employees
  • Create an environment that empowers individuals to take ownership and responsibility for their actions, promote staff initiative and nurture high morale and engagement by embracing strengths-based leadership.
  • Chair the nursing team meetings under the direction of the operations manager and attends other internal/external meetings as appropriate.
  • Develop and implement a department communication plan by conducting market research, selecting appropriate media and message content, coordinating production of all materials to increase public awareness and to provide information regarding new and existing services.

MINIMUM KNOWLEDGE, SKILLS & ABILITIES

  • Bachelor of Science Nursing or equivalent
  • 3 years of related experience, nursing experience is preferred, paediatric rehabilitation experience is an asset
  • Demonstrated Leadership through experience as team leader, Hospital Charge Nurse or other leadership experiences
  • Registered Nurse (RN) in good standing with the College of Nurses of Ontario
  • Member of Registered Nurses' Association of Ontario (RNAO) is preferred
  • Canadian Nursing Specialty Certification such as Pediatrics CpedN(C) Certification in Pediatric Nursing (C)anada is preferred
  • Excellent oral and written communication skills
  • Demonstrated problem solving, risk assessment and critical thinking skills to solve client/family issues.
  • Awareness of current organizational policies and standards of care for Nursing/Respiratory l
  • Demonstrated ability to engage and work collaboratively with all levels and members of an interdisciplinary team
  • Knowledge of family centered care principles and theories/concepts of family systems nursing
  • Knowledge of fair and just patient safety culture
  • Knowledge of organizations that support our clients in the community (e.g., Children's Treatment Centres, LHIN Home and Community Case Coordinators, to assist with transition to community.
  • Experience using technology related to accessing information, e.g. pre-admission, electronic Child Health Network.
  • Awareness of all contingency plans is vital to respond all codes/crisis situations that may occur
  • Strong computer skills and analytical skills
  • Knowledge of collective agreements and working experience in a unionized environment is preferred.

PHYSICAL AND ENVIROMENTAL DEMANDS

  • This position is exposed to regular moderate hazardous conditions when working with objects/equipment which could result in injuries.
  • Work environment has constant flow of people, background noise and interruptions. May be exposed to potential escalated behaviours from people in a controlled setting where there is access to other staff for assistance.
  • It requires regular sensory effort such as visual attention and listening carefully. Physical demands are associated with intermittent standing, walking, and sitting.
