The Position:

The Coordinator, Access & Flow is a new role - designed to be a key member of the Access & Flow team. The team consists of the Appointment Services team and the Inpatient Flow team. Appointment Services is responsible for processing referrals, scheduling clinical appointments, registering clients, and maintaining accurate health records. The Inpatient Flow team is responsible for providing leadership related to planning - for admission into and discharge from inpatient services - and strives to deliver an exceptional client/family experience throughout their health care journey. This is a 12-month contract.

Key Responsibilities:

The Coordinator, Access & Flow, will support both the Appointment Services team and the Inpatient teams. The key responsibilities are as follows:

  • Coordinating with community CTCs (Children's Treatment Center) regarding incoming referrals, and cultivating relationships with CTCs as key external stakeholders
  • Updating a database of services provided by CTCs to display a regional breakdown
  • Coordinating OHaH (Ontario Health at Home) referrals for clients (pre-discharge)
  • Liaising with physicians/ NPs regarding outgoing referrals
  • Coordinating intake meetings between external partners (acute care) and internal clinical team members
  • Coordinating with community partners re: discharge planning into the community with supports - to facilitate successful discharge home from Holland Bloorview
  • Project coordination for the Access & Flow Operating plan 2024-25
  • Assist in creating/ maintaining standard work documents for clinical processes led by the Transition Coordinators
  • Coordinate with families and clinical teams on key meetings/ appointments to be scheduled
  • Participate in quality improvement initiatives, and support project implementation and change management documentation/ communication
  • Support data inputting, and data integrity audits across the client registration workflow
  • Contributes to building and fostering an inclusive, diverse, equitable, accessible and anti-racist environment
  • Other duties as assigned

Hours of work are 7.5 hours per day within business hours of 8:00am-4:30pm as required [e.g. 8:30-4:30pm]. On-site presence is required minimum of 4 days / week.

Qualifications:

  • Post-secondary degree/diploma at the university level preferred, or an equivalent combination of education and experience
  • A minimum of five years project / program coordination experience
  • Experience communicating with caregivers/ families as well as clinician team members
  • Effective communicator with strong organizational skills, influential skills, client and family centred care orientation and commitment to providing quality services
  • Intermediate level computer systems and software that are relevant to the job, i.e. word-processing, Excel spreadsheet, and PowerPoint.
  • Strong project management skills; planning, execution and ongoing review and improvement
  • Previous Meditech software experience an asset
  • Ability to work in an area with a high volume of work, frequent interruptions & noise
  • Demonstrated ability to work effectively in both a team environment & independently
  • Willingness to work collaboratively with others
