Evaluation and Systems Planning Lead Health Promotion (Job ID: 8380)
Situated within the Family Health Team, the Evaluation and Systems Planning Lead supports systems level decision making for the Downtown East Ontario Health Team through collecting, analyzing, and reporting on available population health related data in the region. This role focuses on system planning, data analysis, and evaluation to enhance project development, performance management, and health equity initiatives within the community.
DUTIES & RESPONSIBILITIES:
- Inform development of strategic and project planning based on available data and research from community sources, such and community needs assessments, social service agency reports, and community consultations facilitated by partner agencies
- Collect, analyze and report on data from various sources to inform funding opportunities
- Lead the creation of the written submissions to funding EOIs in collaboration with relevant OHT team members, using relevant data and project management principles and tools to ensure success
- Deliver presentations and reports to Core Group, other DET OHT Working Groups, and Ministry, OH etc. to communicate and demonstrate progress of projects.
- Work with communications network/specialist to develop communication strategy and messaging to OHT partners.
- Provide project management support to the Performance Management & Quality Improvement Working Group and Privacy Working Group.
- Report outputs of work activities to Ontario Health, executive team (core group), team members, funders (granting agencies), and other stakeholders including
- Create mid-year and annual progress and performance reports, collaborative quality improvement plan (cQIP), and annual plan dashboard
- Provide expert advice to project team and Ontario Health Team partners on the development of project specific and broader OHT indicators, dashboards, and/or scorecards
- Support projects to align performance reporting and planning with best practices
- Provide project management support to the Performance Management and Quality Improvement Working Group
- Design and implement a data strategy and business intelligence tool(s), such as scorecards and dashboards, for the evaluation of health care processes, patient outcomes, and population health
- Plan and execution of all aspects of data analysis including developing analytic plans, creating documentation and implementing standard processes for the above to improve quality and timeliness of reporting.
- Interpret, synthesize, and present findings for internal and external stakeholders using data visualization, infographics, and storytelling
- Identify opportunities to improve measurement and reporting capabilities of the OHT, through strategic use of Ontario's health system data holdings,
- Respond to evolving information needs either internally or externally driven, and develop strategies for sustainability as needed
- Serves as a subject matter expert on data standards and data integrity
- Provide in-depth data analyses to support quality improvement, performance management, initiatives, planning, etc.
- Provide performance related monitoring and benchmarking in the context of OHT priorities and broader health care system, applying an equity lens
- Develop standardized client experience and outcome measurement tools for all internal stakeholders
- Co-identify meaningful measures/indicators of success that demonstrate improved health outcomes, health equity and client and community experience
- Performs cross functional and other duties as assigned and/or requested.
QUALIFICATIONS:
- Masters' degree in a relevant subject, ex. Informatics, Epidemiology, Health Administration, Public Health, Computer Science required.
- 5 to 7 years relevant work experience in project management, decision support, informatics, in a hospital, community or primary care environment with a track record of timely project completion
- Demonstrated ability to establish and maintain engaged relationships with stakeholders across diverse teams, from community to hospitals, frontline staff to senior leadership
- Demonstrated ability to build consensus through active listening and facilitation and team building skills
- Proficient in the development of a high volume of compelling reports and presentations, effectively communicating with a variety of audiences, including senior management, and stakeholders
- Ability to analyze data and present complex information in an easy to understand manner to inform practice, programming, and decision making
- Adept at designing KPIs that align with organizational goals, ensuring measurable outcomes that reflect the strategic objectives of the organization.
- Proficient in analyzing data sources and identifying relevant metrics, enabling the creation of KPIs that are specific, measurable, achievable, relevant, and time-bound (SMART).
- Experienced in working with cross-functional teams to understand various departmental needs and perspectives, ensuring the developed KPIs are comprehensive and actionable across the organization.
- Demonstrated experience in actively championing and participating in initiatives that focus on diversity, inclusion and equity and embed anti-discrimination principles in service delivery.
- Skilled in utilizing Health Equity databases and municipal health and sociodemographic datasets to inform local population and community health planning.
- Ability to accurately synthesize diverse databases to develop evidence-based assessments and recommendations for population health and community health planning.
- Creative and proactive in identifying innovative approaches to address data gaps, especially for capturing data on priority populations within the DET OHT.
- Excellent understanding of Ontario's health and reporting requirements, particularly as they relate to OHTs, Health Equity, and population health,
- Extensive experience in application of Ministry of Health, OH, and CIHI clinical and administrative data holdings including Population Performance and Utilization Measures, Attributed Populations, Census Dissemination Area data, Costs by Care Type and HPG, Expenses, and Primary Care Data Reports.
- Highly proficient in Microsoft Office Suite products, database and analysis software applications such as SQL, PowerBI, Tableau, Python, or R.
