Health Care Attendant - Transitional Care (Job ID: 8924)
Position Summary:
The Health Care Attendant (HCA) will provide high quality patient care within an Interprofessional Collaborative Practice philosophy. As part of a multi-disciplinary team receiving functional supervision from the RN/RPN, the clinical assistant will be responsible for contributing to patient care by providing basic patient support and for assisting with indirect care tasks. Duties will include:
- Contributing to patient profile upon admission by supporting the patient upon entry
- Assisting patients with activities of daily living including proper hygiene care
- Providing one-to-one constant patient care as required
- Assisting patients adapt to the use of various assistive devices, specifically: fitting and teaching patients on crutch techniques
- Answering call lights and arranging for the proper safekeeping of patients' belongings
- Demonstrating the ability to show initiative, independence, priority setting and problem solving techniques in a time limited and chaotic environment, while maintaining composure
- Participating, with team members, in the patient care planning process and in the development and implementation of quality-based initiatives
Qualifications:
- Health Care Aide (HCA) or Personal Support Worker (PSW) diploma certificate from a recognized program approved by the Ontario Ministry of Training, Colleges and Universities that is no less than 625 hours in length
- Current certification in Basic Lifesaver, Basic Rescuer C or Heartsaver Plus
- Recent acute care experience
- Demonstrates excellent interpersonal and communication skills, both verbal and written in the English Language
- Ability to perform FULL range of duties in this position including specific types of patient care as directed within the list approved for the category and by the Regulated Health Professions Act (RHPA)
- Ability to provide portering and transportation services for patients and supplies
- Ability to rotate on day, evening and night shifts as scheduled
- Satisfactory attendance record
