Infection Control Practitioner (ICP)

Temporary Full Time-15 Month Contract

The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church & Bloor St) and our Harbour Lights site and Specialized Care Centre (Victoria Park & McNicoll). Position Summary:

Reporting to the Director, Quality, Risk & Patient Safety, the Quality and Patient Safety Specialist will work closely with all departments and levels within the hospital to provide assistance on IPAC related projects and initiatives and implements various plans. This is an exciting time to join The Toronto Grace and make a big impact in our Specialized Care Centre and Harbour Light locations at 1020 McNicoll Avenue.

Responsibilities:

This position will be required to work at all Toronto Grace Sites (SCC, Harbour Light and 650 Church St.)

In conjunction with IPAC Team and departmental Director implements projects related to infection prevention and control

Provides various assistance and support to departmental Director and Management team related to IPAC deliverables

Conducts regular surveillance for potential outbreaks in collaboration with Public Health

Assists with surveillance processes for screening and surveillance for antibiotic resistant organisms (AROS) and C-difficile associated disease (CDAD)

Monitors and tracks patient data to assess risk of respiratory and enteric outbreaks

Create more awareness of infection control related patient safety issues for staff at all levels

Implements guidelines for safe environment practices related to potential enteric and respiratory outbreaks

Provides support on various investigations and outbreak management

Create more awareness for staff at all levels regarding risk of transmission

Coordinates staff and patient education programs such as the infection control programs on hand washing and prevention of hospital acquired infections

Delivers various staff presentations on relevant and timely topics

Other related duties as deemed necessary

Qualifications and Education:

Baccalaureate preparation in a relevant health-science discipline

Completion of the Infection Prevention and Control Practitioner Course

Board certification in infection control (considered an asset)

Knowledge and experience in infection control theory and practice

Experience working in a hospital setting

Project Management experience an asset

Excellent customer service skills

Excellent time management skills

Exercise initiative and good judgment with ability to multitask

Strong organizational and analytical skills

Demonstrated ability to work within a team environment

Skilled in the use of spreadsheets and computer databases

Strong interpersonal and communication skills

Ability to provide staff with education as necessary

Compliance with confidentiality requirements

Microsoft Office skills – Word, Power point, Excel
