A division of the Primary and Community Based Care portfolio, the Clinical Transformation (CT) Team advances integrated care for patients and their caregivers through the implementation and operation of virtual care programs, resources and supports. We endeavor to improve outcomes, have a positive impact on patient and provider experiences and demonstrate value for investment.

As a member of the Provincial RCM Programs team, the Lead champions the transformation of health care through the planning, design, delivery and evaluation of new models of care, aligned with best practices and enabled by technology. Work includes leading co-design, expansion, and evolution of provincial, regional, and local virtual care programs in alignment with population health management priorities. The Lead, RCM has extensive knowledge and experience in virtual care models, implementation and adoption, and has built effective relationships with multiple and diverse stakeholders at all levels.

Working within a matrixed team that includes Strategy, Analytics, Quality, and Engagement team members, the Lead, RCM plays an integral role in the spread and scale of RCM in Ontario. This role will collaborate closely with internal OH teams as well as external stakeholders such as Ministry of Health (MoH), Ontario Health Teams (OHTs), healthcare organizations (HCOs), vendors, health care providers (HCPs), and patients, to test innovative clinical models/programs and scale leading models/programs aligned with strategic priorities and objectives.

Here is what you will be doing:

  • Work collaboratively with OHTs/HCOs, OH Region Digital and Clinical Team Members and other HCPs/stakeholders to identify and enable virtual care models, programs and solutions that enable transformation and that meet partners’ and patient needs

  • Act as a SME to provide expert advice to internal and external partners in relation to virtual care policies, care models, programs, initiatives, aligned with best practices from Ontario and other jurisdictions

  • Work collaboratively with OH Regional teams, external partners and the MoH to secure funding, develop funding guidelines and provide recommendations on allocation of provincial virtual care funding to OHTs and other eligible HCOs for new and innovative models of care

  • Works with HSPs to ensure accountability to funding agreements

  • Work directly with partner organizations to lead/enable project planning, adoption, implementation, change management, evaluation, and monitor progress of funded initiatives in collaboration with Ontario Health Regions

  • Facilitates meetings and conducts effective partner consultation to assess and determine readiness for RCM implementation

  • Work directly with partner organizations to lead/enable project planning, adoption, implementation, change management, evaluation, and monitor progress of funded initiatives in collaboration with Ontario Health Regions.

  • Lead and participate in internal and external committees to continually assess and advance the delivery and quality of virtual care

  • Work with partners, funders, and users to identify and facilitate scaling of successful projects

  • Continuous learning on trending virtual care products and services, digital healthcare solutions, related privacy and security issues, and MoH policy

  • Creation of clear and effective meeting materials, knowledge products, etc. to lead partner engagements

  • Lead and support matrixed projects with a focus on virtual care, as required

  • Other duties as assigned

Here is what you will need to be successful:

Education and Experience

  • Bachelor's degree in health-related discipline, business administration or in a field related to the job responsibilities or equivalent work experience

  • Minimum 7-10 years of business development, advisory, or project management experience, preferably in a health-related field

  • Project management experience in health system planning and supporting/enabling large-scale strategic projects

  • Experience leading development, implementation and providing oversight of multiple and diverse innovative programs/projects and/or virtual care models and technologies concurrently, in complex clinical environments

  • Experience in coaching/collaborating with program/project delivery teams of various sizes and scope (may include staff, consultants, and vendors)

  • Experience managing accountable stakeholder relationships; establishing and monitoring contracts

  • Experience in data analysis, reporting, project evaluation and knowledge translation

Knowledge and Skills

  • Strong understanding of Ontario’s health care system and the overarching provincial health care strategy (including Digital First for Health Strategy), healthcare environment, including HCPs and support team, their roles, their interactions, and challenges

  • Deep understanding of virtual care models, solutions, and platforms

  • A strong leader, with proven ability to build, enhance and maintain strategic relationships with health care leaders, provincial government, OHTs, HCPs and other key stakeholders

  • Capacity to think strategically, identify and articulate opportunities that align with OH priorities, and maintain broad systems view to guide future planning and operations

  • Experience influencing leadership with strategic vision through data-driven analysis and thoughtful presentation of recommendations

  • Ability to work independently, with guidance only for the most complex situations.

  • Strong presentation skills to lead effective and productive meetings

  • Excellent computer skills required (e.g., Microsoft Word, Excel and PowerPoint, SharePoint)

  • Knowledge of current trends in global virtual care; a learning posture to support the spread of successful models and best practices across Ontario
