Manager, Homeless and Outreach Services - Emergency (Job ID: 7051)

This newly created role, Manager of Homeless and Outreach Services, will provide dedicated leadership and oversight for homeless and outreach programs within the Emergency Care and Medicine programs, and make connections to initiatives for homeless and vulnerably housed individuals across the organization and with community partners and funders. This individual will enhance the hospital's response to the needs of our inner-city population by focusing on social determinants of health, such as homelessness, substance use, poverty, and the needs of newcomers. The manager will be crucial in advocating for patients, and will reflect their concerns and ideas in strategic planning, and foster strong community engagement.

Responsibilities:

  • Working alongside the Emergency Care and Medicine management teams and reporting directly to the Senior Clinical Program Director, this individual is responsible for:
  • Providing expertise in the areas of homelessness, outreach, community engagement, and equity;
  • Developing and building relationships with internal and external stakeholders/ communities;
  • Advocating on behalf of patients to ensure representation of the patient and community voice in strategic planning and other departmental activities;
  • Developing strategic objectives for health equity and community engagement in a manner that builds upon our UHT vision;
  • Intimate understanding of roles, programs and practices within the Department and Team to effectively build relationships and assure relevancy for a diverse workforce.
  • Maintains responsibility for developing and executing human resources strategies for attracting, hiring, managing, developing and motivating union and non-staff
  • Provides direction/leadership for the team as a coach and mentor; defines accountabilities while directing and supporting the achievement of performance goals, within functional area of responsibility


Qualifications:

  • University Degree in Health / Social Services or related field with a minimum of 5 years of related work experience;
  • Master's Degree in the above preferred
  • Demonstrated leadership and knowledge/skills in population health approaches, health equity, health policy, knowledge exchange, applied research, program planning, evaluation, and population health assessment and surveillance. Specialized knowledge and demonstrated leadership in the area of health equity and social inequities in health;
  • Proven project/program management, interpersonal and administrative skills, including the ability to set priorities, establish timeframes, and organize resources;
  • Strong strategic, analytical, problem-solving, facilitation and negotiation skills, as well as systems thinking ability;
  • Experience in developing and supporting collaborative relationships with different groups or agencies;
  • Excellent communication (verbal and written) skills, particularly with respect to the interpretation and presentation of evidence and in the mobilization of project teams;
  • Demonstrated computer skills, including experience with qualitative and quantitative data analysis and software;
  • Demonstrated interpersonal skills, including collaborating with a team, working across program areas, disseminating information effectively and conflict resolution;
  • Experience interacting with individuals from diverse ethnic and socio-economic backgrounds;
  • Proven strong understanding and commitment to the social determinants of health;
  • Knowledgeable of key change management and organizational development/ change principles (particularly with respect to equity and diversity);
  • Demonstrated experience in the coordination of events and public engagement, communication and/or workshop facilitation;
  • Demonstrated commitment to the mission and values of Unity Health
