The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church & Bloor St) and our Harbour Lights site and Specialized Care Centre (Victoria Park & McNicoll). Position Summary:

Reporting to the Chief Nurse Executive the Manager of Quality, Risk and Patient Experience will be responsible for providing leadership, development and delivery of TGHC Quality, Patient Experience and Safety plans and processes. The position is accountable for monitoring the organizations systems, processes and procedures for quality, patient experience and safety and works closely with the management team and staff to create a culture of safety and ongoing learning using an integrated risk based approach to manage emerging issues, identified incidents and control process weaknesses.

Responsibilities:

Ensures that designated contractual obligations between the hospital and stakeholders are met (e.g. quality improvement plan, patient experience framework implementation, quality dashboards).

Participates in developing, implementing and maintaining strategic plans and ensuring corporate alignment.

Develop tools to support decision making and provide system-wide support to promote and influence a culture of safety and enhance quality of care through data driven analyses (e.g. failure modes and effects analysis (FMEA), human factors, LEAN planning, statistical process control).

Supports operational leadership to develop and implement organizational strategic quality improvement projects and evaluation frameworks consistent with the goal of a high reliability organization.

Implements patient experience framework and strategy to ensure integration of patient voice in organizational decision-making

Provides regular reports (briefing notes, dashboards, presentations) to update the various levels of staff and leadership on quality improvement/patient safety initiatives.

Provides operational leadership for quality, patient safety, and overall patient experience.

Develops and implementing corporate risk management framework, policies and procedures

Provides frontline leadership, collaboration and direction in the event of a patient safety incident and providing subject matter expertise to program leadership regarding reengineering the system to mitigate risk (e.g. system reviews, disclosures, integrating patient safety best practice and subject matter expertise).

Guides the organization in meeting regulatory and legislative requirements

Advises senior management decision-makers on process improvements and systems required to balance risks, opportunities, staff and patient safety, revenues, and costs.

Leads the hospital accreditation process

Delivers education in-services on quality improvement, patient safety, and patient experience technical concepts (e.g. human factors, lean, process mapping, prospective risk analyses, and service design, evidence based co-design with patients and families).

Other duties as assigned

Qualifications and Education:

Undergraduate degree in a Health related discipline.

Master’s degree in Quality Improvement & Patient Safety, Health Administration or Business Science preferred

Minimum ten years of progressive leadership experience in a healthcare facility preferably in quality improvement, risk management or senior health operations

Certification in Risk Management and Quality Improvement an asset

Patient Safety Certification

Knowledge of MS Office (Outlook, Word, Excel, PowerPoint, Access, Visio, etc.) Healthcare systems

Proficient knowledge of legislation and regulatory environment applicable to hospitals/healthcare, including Excellent Care for All Act, Quality of Care Information Protection Act and Privacy Act

Knowledge of risk reduction/management principles, service design (e.g. patient experience), quality improvement science (e.g. LEAN, Six Sigma, etc.) and Change management

Knowledge of High Reliability Organization (HRO) tactics and initiatives

Tactful and diplomatic with the ability to negotiate, manage conflicting demands and solve problems.

Excellent verbal and written communication skills

Excellent attendance record
